多发性硬化患者外周血调节性B细胞等相关B淋巴细胞的研究

【摘要】：目的通过检测多发性硬化患者与正常对照组调节性B细胞等相关B淋巴细胞占总B淋巴细胞百分率差异,并且分析患者不同临床严重度与调节性B细胞的相关性,探讨各B淋巴细胞亚群在多发性硬化中的潜在作用。方法收集从2016年7月到2017年3月入蚌埠医学院第一附属医院神经内科住院的多发性硬化患者(n=12,男8例,女4例,年龄41.42±4.53岁)及同期门诊体检的无神经系统或免疫系统疾病的健康者(n=12,性别、年龄均与多发性硬化组匹配)外周静脉血样本,并对所有多发性硬化患者进行EDSS伤残评分。采用流式细胞仪检测患者及对照组外周静脉血中CD19+CD24hi CD38hi细胞、CD19+CD27-CD38hi细胞、CD19+CD27hi CD38hi细胞占B淋巴细胞总数的百分率,酶联免疫吸附测定患者及对照组外周静脉血中IL-10水平。分析比较两组的B淋巴细胞亚群差异及MS组CD19+CD24hi CD38hi细胞比例与EDSS是否具有线性相关。结果多发性硬化患者外周血调节性B细胞(CD19+CD24hi CD38hi)及过渡型B细胞(CD19+CD27-CD38hi)占总B淋巴细胞的百分率及IL-10水平与对照组之间差异有统计学意义(P0.05),多发性硬化患者外周血调节性B细胞、过渡型B细胞及IL-10水平均降低。多发性硬化患者浆母细胞(CD19+CD27hi CD38hi)占总B淋巴细胞的百分率与对照组相比无统计学意义(P0.05)。临床严重性EDSS评分不同的多发性硬化患者与其调节性B细胞表达率无统计学差异(P0.05)。结论MS患者发病期调节性B细胞及过渡型B细胞表达降低伴随IL-10减少,B淋巴细胞参与多发性硬化的发病。调节性B细胞表达降低与临床严重度可能无关。
[Abstract]:Objective to detect the percentage of regulatory B lymphocytes in the patients with multiple sclerosis (MS) and normal controls, and to analyze the correlation between the clinical severity and the regulatory B cells in patients with multiple sclerosis. To explore the potential role of B lymphocyte subsets in multiple sclerosis. Methods from July 2016 to March 2017, 12 patients with multiple sclerosis, 8 males and 4 females, were admitted to the Department of Neurology, first affiliated Hospital of Bengbu Medical College. Age 41.42 卤4.53 years old) and healthy subjects with no neurological or immune system diseases in the same period of outpatient examination, sex and age matched with multiple sclerosis group) peripheral venous blood samples were collected, and all patients with multiple sclerosis were evaluated with EDSS disability score. The percentage of CD19 CD24hi CD38hi cells, CD19 CD27hi CD38hi cells in peripheral venous blood of patients and control group was detected by flow cytometry, and the level of IL-10 in peripheral vein blood of patients and control group was measured by enzyme-linked immunosorbent assay (Elisa). The difference of B lymphocyte subsets between the two groups and whether the proportion of CD19 CD24hi CD38hi cells in MS group were linearly correlated with EDSS were analyzed and compared. Results the percentage of CD19 CD24hi CD38hiand the percentage of CD19 CD27-CD38hi1 in total B lymphocytes and the level of IL-10 in peripheral blood of patients with multiple sclerosis were significantly different from those of the control group (P 0.05). The levels of transitional B cells and IL-10 were decreased. The percentage of CD19 CD27hi CD38hi1 in total B lymphocytes in patients with multiple sclerosis was not significantly higher than that in control group (P 0.05). There was no significant difference in the expression rate of regulatory B cells between multiple sclerosis patients with different clinical severity EDSS scores (P 0.05). Conclusion the decreased expression of regulatory B cells and transitional B cells in MS patients is associated with the decrease of IL-10 B lymphocytes in the pathogenesis of multiple sclerosis. The decrease of regulatory B cell expression may not be associated with clinical severity.
