原发性脑出血短期临床预后分析

【摘要】:背景:虽然原发性脑出血(ICH)在全部卒中类型中所占比例较缺血性卒中低,但却与高的致死率和致残率相关,易造成永久性的神经功能障碍。前期研究对影响脑出血远期预后的因素进行了较多报道,但对短期预后的报道较少。 目的:探讨原发性脑出血患者短期预后的影响因素。 方法:回顾性分析我院235例发病后在60天内出院的急性原发性脑出血患者的既往史、临床、影像学、实验室数据、内科或外科治疗、并发症等相关资料,通过MRS评分评价患者出院时的预后情况。采用多因素Logistic回归方法分析与短期预后独立相关的因素。 结果:共235名原发性脑出血患者,出院时预后良好组为128例(54.5%),预后不良组为107例(45.5%)。在多因素Logistic回归分析中,年龄(OR=1.071,p0.001)、GCS评分(OR=0.755,p=0.024)、GCS≤8分(OR=0.101,,p=0.013)、入院时NIHSS评分(OR=1.169,p=0.001)、出血破入脑室(OR=3.356, p=0.003)、中线结构移位(OR=2.894,p=0.014)是短期预后的影响因素(p0.05)。 结论:45.5%的原发性脑出血患者在发病60天内出院时预后不良;年龄、GCS评分(GCS评分≤8分)、入院时NIHSS评分、出血破入脑室、中线结构移位是影响原发性脑出血患者短期预后的独立危险因素。
[Abstract]:Background: although the proportion of primary cerebral hemorrhage (ICH) in all stroke types is lower than that in ischemic stroke, it is associated with high mortality and disability rate and is prone to permanent neurological dysfunction. Previous studies have reported many factors affecting the long-term prognosis of intracerebral hemorrhage, but few have been reported on short-term prognosis. Objective: to explore the influencing factors of short-term prognosis in patients with primary cerebral hemorrhage. Methods: the history, clinical, imaging, laboratory data, internal or surgical treatment and complications of 235 patients with acute primary cerebral hemorrhage who were discharged from our hospital within 60 days after onset were retrospectively analyzed. The prognosis of patients at discharge was evaluated by MRS score. Multivariate Logistic regression was used to analyze independent factors associated with short-term prognosis. Results: of 235 patients with primary cerebral hemorrhage, 128 (54.5%) had a good prognosis and 107 (45.5%) had a poor prognosis.
