人脑胶质瘤中CD133、SSEA-1和Nestin的表达和意义

【摘要】:目的:胶质瘤是脑肿瘤中最常见的肿瘤,约占50%。尽管目前的外科手术,,放、化疗都有了极大的提高,但是对于胶质瘤的治疗及预后,还远未达到到理想的效果。胶质瘤干细胞的提出,给我们提供了一个全新的思路。本实验欲研究胶质瘤干细胞标志物CD133、SSEA-1(阶段特异性胚胎抗原-1)和Nestin(神经巢蛋白)在人脑胶质瘤组织中表达及与病理分级的相关性;探讨三者在人脑胶质瘤的诊断及恶性程度判断中的临床意义,以及为治疗提供分子生物学理论基础。 方法:收集广西医科大学一附院(2011年9月—2012年12月)经病理证实为胶质瘤,按2007年WHO分级标准:Ⅰ级6例,Ⅱ级16例,Ⅲ级13例,Ⅳ级19例,另外用6例正常脑组织为对照。应用免疫组织化学的方法检测54例脑胶质瘤组织和6例正常脑组织标本中CD133、SSEA-1及Nestin的表达。 结果:①54例脑胶质瘤组织和6例正常脑组织标本中CD133阳性细胞率率分别为25.38%和0%(P0.05),SSEA-1阳性细胞率分别为26.62%和0%,Nestin阳性细胞率分别为22.39%和0%。②对54例脑胶质瘤组织中CD133和SSEA-1及Nestin阳性细胞率分别进行多个病理级别之间的均数比较: I级和Ⅱ级之间差别无统计学意义(P>0.05),Ⅰ级和Ⅲ、Ⅳ级间差别具有统计学意义(P<0.01),Ⅱ级和Ⅲ、Ⅳ级间差别具有统计学意义(P<0.05), Ⅲ级和Ⅳ级间差别具有统计学意义(P<0.05)。③脑胶质瘤组织中SSEA-1阳性细胞率大于CD133大于Nestin,差别具有统计学意义(P<0.05);SSEA-1阳性细胞率和CD133阳性细胞率呈正相关关系(Cp=0.84, P<0.05);CD133阳性细胞率和Nestin阳性细胞率呈正相关关系(Cp=0.48,P<0.05);SSEA-1阳性细胞率和Nestin阳性细胞率呈正相关系(Cp=0.44,P<0.05)。 结论:①人脑胶质瘤组织中在不同病理级别的胶质瘤标本组中CD133、SSEA-1及Nestin表达程度不同; CD133、SSEA-1、Nestin的表达与胶质瘤病理分级呈正相关,提示三者的高表达与胶质瘤的恶性程度有关。②检测脑胶质瘤组织中CD133、SSEA-1、Nestin的表达有助于胶质瘤患者的诊断及恶性程度判断。③CD133、SSEA-1和Nestin作为胶质瘤干细胞标记物在胶质瘤的诊断和治疗以及评估预后和未来个性化综合治疗中发挥重大的作用。

Although the current surgery, radiotherapy and chemotherapy have been greatly improved, but for the treatment and prognosis of glioma, it is far from the ideal effect. The proposal of glioma stem cells provides us with a new way of thinking. The purpose of this study was to study the expression of glioma stem cell markers CD133,SSEA-1 (stage specific embryonic antigen-1) and Nestin (nestin) in human glioma tissues and their correlation with pathological grade, to explore the clinical significance of the three markers in the diagnosis and malignant degree of human glioma, and to provide the theoretical basis of molecular biology for treatment. Methods: from September 2011 to December 2012, glioma was confirmed by pathology in the affiliated Hospital of Guangxi Medical University.
