输尿管部分梗阻动物模型及其形态学研究

【摘要】: 目的: 建立一种较为理想的输尿管部分梗阻动物模型;探索动物模型肾血管铸型的方法;观测比较正常肾与输尿管部分梗阻动物模型积水肾的形态学变化,为肾积水的相关研究提供形态学资料。 方法: 健康成年新西兰兔48只,进行以下处理: 1. 12只作为正常对照组(N)。 2. 36只兔采用改进的输尿管套管法,建立左侧输尿管部分梗阻动物模型,作为梗阻观察组(O),N组及O组梗阻术后1周、2周、4周(即O1、O2、O4)各随机选取6只,分别行B超、静脉肾盂造影。 3. N组及O1、O2、O4组各随机选取6只兔以过氯乙烯、牙托粉为填充剂分别行肾动脉、肾盏和肾盂铸型,测定肾动脉、肾干动脉、肾段动脉、叶间动脉管径大小;各组分别选取6只作组织学切片,了解输尿管部分梗阻后肾形态学变化规律。 结果: 1.随梗阻时间延长,肾盂前后径(APD)及肾盂容积(V)进行性增大,O组各期与N组比较差异有显著性意义(P0.01);肾皮质厚度(RCT)则逐渐减小,O1、O2组与N组比较差异无显著性意义,O4组明显变薄(P0.01)。 2.随梗阻时间延长,静脉肾盂造影示肾大小盏、输尿管明显扩张,O组各期肾长径及肾横径与N组比较差异有显著性意义(P0.01)。 3.兔左或右肾动脉都为一条,其一级分支以前、后干的二干型为多,前、后干分别于肾盂前后面的肾实质形成肾盂前、后动脉系,肾干动脉发出口径较细的二级分支即肾段动脉,前干通常分为前上、前中、前下段动脉,分布于相应肾盂前面的肾实质;后干通常分为后上、后中上、后中下及后下段动脉,分布于相应肾盂后面的肾实质。N组肾动脉及其各级分支在肾实质内分布密集、均匀,随梗阻时间延长和程度的加重,血管分布稀疏、变形。肾动脉O组各期与N组管径比较差异无显著性意义;肾干动脉、肾段动脉O4组与N组比较差异均有显著性意义;叶间动脉O2、O4组分别与N组比较差异均有显著性意义,且叶间动脉O4与O2组比较差异有显著性意义。N组肾盂、肾盏轮廓清晰,杯口锐利,无扩张,随梗阻时间的延长,O组肾大小盏逐渐扩张,肾盏杯口渐渐变钝、变平,肾盂逐渐扩张、变形。 4.组织学切片显示O1组:肾小球、肾小管结构基本正常,尚无显著改变;O2组:肾小球轻度萎缩,肾小管部分扩张,肾间质炎症细胞浸润和成纤维细胞浸润增多;O4组:肾小球明显萎缩,肾小囊扩张,肾小管明显扩张,管内可见胶样蛋白液体沉积,血管扩张充血,肾间质炎症细胞和成纤维细胞浸润明显,肾间质部分区域坏死钙化。 结论: 1.改良的输尿管套管法建立的输尿管部分梗阻动物模型,方法简便、操作容易、效果良好,完全符合输尿管不全梗阻的要求,是一种较为理想的输尿管不全梗阻动物模型。 2.以过氯乙烯、牙托粉分别作为肾血管及肾盂、肾盏的灌注填充剂,灌注效果好,结构显示清晰、连续、完整,是良好的灌注材料,灌注方法简便,能达到预期效果,值得推广。 3.肾动脉、肾动脉前后干、肾段动脉管径在梗阻期变化不明显,叶间动脉管径明显缩小。
[Abstract]:Purpose :
To establish an ideal animal model of ureter partial obstruction ; to explore the method of animal model of renal vascular cast ; to observe the morphological changes of the kidney in the animal model with normal kidney and ureter obstruction , and to provide morphological data for the study of the related study of renal hydrops .
Method :
The following treatments were performed for 48 healthy adult New Zealand rabbits :
1.12 As the normal control group ( N ) .
2.Thirty - six rabbits were treated with modified ureteric cannula method to establish an animal model of left ureteral obstruction . As an obstruction observation group ( O ) , N group and O group , 6 rats were randomly selected for 1 week , 2 weeks and 4 weeks ( i.e . O1 , O2 , O4 ) .
3 . Six rabbits were randomly selected in group N and O1 , O2 and O4 . The renal artery , renal artery , renal artery and interlobe artery diameter were measured by using perchlorethylene and denture powder as filling agent , and 6 rabbits were respectively selected for histological sections to understand the morphological changes of renal morphology after partial obstruction of ureter .
Results :
1 . With the prolongation of the obstruction time , the anterior and posterior diameter of the renal pelvis ( APD ) and the volume of renal pelvis ( V ) increased , there was significant difference between the O group and the N group ( P0.01 ) . The thickness of the renal cortex ( RCT ) decreased gradually , and there was no significant difference between the O1 and O2 groups and the N group , and the O4 group was significantly thinner ( P0.01 ) .
2 . With the prolongation of the obstruction time , the size of the kidney was shown by the intravenous pyelography , the ureter was obviously dilated , the renal length and the renal transverse diameter in the O group were significantly different from that of the N group ( P0.01 ) .
There was no significant difference between the renal parenchyma and the renal parenchyma of the renal artery and the renal artery .
4 . The histological sections showed that there were no significant changes in the structure of glomerulus and tubular tubules , but there was no significant change in the structure of glomerulus and tubular tubules .
Conclusion :
1 . The modified animal model of ureteral obstruction established by the modified ureter cannula method is simple , easy to operate , good in effect , fully compliant with the requirement of ureteral obstruction , and is an ideal animal model for ureteral obstruction .
2 . The perchloride and denture powder are used as the filling agent for the renal blood vessels and the renal pelvis and the kidney , the perfusion effect is good , the structure is clear , continuous and complete , is a good perfusion material , the perfusion method is simple and convenient , the expected effect can be achieved , and the method is worthy of popularization .
3 . There was no obvious change in the diameter of the renal artery and the renal artery before and after the renal artery .
