重症极外侧型腰椎间盘突出症患者的椎间孔镜下手术效果

【摘要】：目的探讨重症极外侧型腰椎间盘突出症患者的椎间孔镜下的手术效果。方法用孔镜下腰椎间盘切除术(Percutaneous Endoscopic Lumbar Discectomy,PELD)治疗50例重症极外侧型腰椎间盘突出症患者;记录患者的手术时间、出血量、住院时间,比较术前、术后12周、术后24周和一年时的视觉模拟疼痛评分(visual analogue scale,VAS)、功能障碍指数(Oswestry disability index,ODI)评分,使用改良Macnab标准评价临床疗效。结果患者的疗效优良率为92.0%;患者在手术前、术后12周、术后24周及术后1年腰痛、腿痛的VAS评分和ODI评分改善显著(P0.01)。结论椎间孔镜下腰椎间盘切除术治疗重症极外侧型腰椎间盘突出症,疗效显著,安全性高。
[Abstract]:Objective to investigate the surgical effect of transforaminal endoscopic disc herniation under severe extralateral lumbar. Using the method of hole endoscopic lumbar discectomy (Percutaneous Endoscopic Lumbar Discectomy, PELD) in treatment of 50 cases of extreme lateral lumbar disc herniation patients; record the operation time, bleeding volume and hospitalization time and the comparison of preoperative, postoperative 12 weeks, 24 weeks after operation and one year when the visual analogue score (visual analogue scale VAS (Oswestry disability), index disability index, ODI) score, clinical efficacy evaluation using modified Macnab criteria. The results were excellent and good rate was 92%; the patients before surgery after 12 weeks, 24 weeks after surgery and postoperative 1 years back pain, leg pain VAS score and ODI score improved significantly (P0.01). Conclusion transforaminal endoscopic discectomy for the treatment of severe extreme lateral lumbar disc herniation, efficacy, safety High.

【正文快照】： 腰椎间盘突出症是现代临床较为常见的疾患,主要是由于腰椎间盘各部分,包括髓核、纤维环、软骨板,出现不同程度的退行性改变,其中以髓核的改变最为严重,发病率较高的部位为腰椎4~5、腰5-骶1位置,约占95%[1],门诊的就医患者以腰腿疼痛为表现,其中腰椎间盘突出症所占比例为36%[2]
