不同剂量谷氨酰胺肠内营养对重度颅脑损伤患者血浆二胺氧化酶和D-乳酸的影响

【摘要】:目的探讨不同剂量谷氨酰胺肠内营养对重度颅脑损伤患者血浆二胺氧化酶(DAO)和D-乳酸的影响。方法将安徽省立医院南区2015年2月至2015年12月神经外科住院的60例重度颅脑损伤患者以完全随机化法分成3组:不含谷氨酰胺肠内营养组(C组),低剂量0.3 g/(kg·d)谷氨酰胺肠内营养组(L组),高剂量0.6 g/(kg·d)谷氨酰胺肠内营养组(H组),每组20例。于肠内营养支持的第1天、第8天、第15天晨空腹采血,检测血浆DAO及D-乳酸水平,并比较其变化。结果 3组患者第1天血浆DAO及D-乳酸水平变化不明显,差异无统计学意义(P0.05)。在第8天、第15天,L组和H组患者血浆DAO及D-乳酸水平低于C组,差异均有统计学意义(P0.05)。结论添加谷氨酰胺肠内营养有助于改善重度颅脑损伤患者的肠黏膜功能,且随着谷氨酰胺剂量增加,保护作用加强。
[Abstract]:Objective to investigate the effects of different doses of glutamine enteral nutrition on plasma diamine oxidase (diamine oxidase) in patients with severe craniocerebral injury. Methods from February 2015 to December 2015, 60 patients with severe craniocerebral injury in the Southern District of Anhui Provincial Hospital were randomly divided into 3 groups:. No glutamine enteral nutrition group (. Group C). Low dose 0.3 g / kg / d) glutamine enteral nutrition group (group L) and high dose group (0.6 g / kg / d) glutamine enteral nutrition group (group H). Fasting blood samples were collected from 20 patients in each group on day 1, day 8 and day 15 of enteral nutrition support. Plasma levels of DAO and D-lactic acid were measured. Results there was no significant change in plasma DAO and D-lactic acid levels in the three groups on the first day, but there was no significant difference (P 0.05). On the 8th day and 15th day, there was no significant difference in plasma DAO and D-lactic acid levels in the three groups. The plasma levels of DAO and D-lactic acid in group L and group H were lower than those in group C. Conclusion addition of glutamine enteral nutrition can improve the intestinal mucosal function in patients with severe craniocerebral injury, and the protective effect is enhanced with the increase of glutamine dosage.
