Nanofat在面部非结构性移植中的临床观察

【摘要】:目的:第一部分观察评价Nanofat移植在面部非结构性移植中的临床效果第二部分借鉴谭新颖等建立的可注射自体脂肪移植的动物模型建立Nanofat非结构性移植的动物实验模型。方法:第一部分2014年9月至2014年12月,收集10例自愿来我院进行脂肪移植治疗女性患者,其中面颊部皮肤老化1例,眼周皮肤老化5例,额部皮肤老化2例,瘢痕2例。常规吸脂术获取脂肪,机械乳化后留Nanofat,采用27号针头真皮深层注射,直到皮肤颜色发生变化为止,注射后按疗程定期来我院随访拍照及接受后续治疗。第二部分从6只新西兰家兔背部肩胛间区或腹股沟区获取脂肪组织,取部分脂肪颗粒经过机械乳化处理后作为实验组,移植到兔的右耳移植区,未经过处理的为对照组移植到兔左耳移植区。术后当日,1月,2月对两组脂肪成活率及对移植区域皮肤的影响。结果:第一部分有效9例,其中皮肤老化明显改善7例,无效1例,无严重并发症;随访期内效果稳定7例,再手术3例第二部分可注射脂肪的兔耳Nanofat非结构性移植的动物实验模型易于观察和活体测量,容易重复的优点。结论:第一部分Nanofat移植有助于面部皮肤年轻化。第二部分Nanofat经过机械乳化处理破坏了脂肪的组织结构及失去细胞活性,影响了脂肪移植的成活率,但刺激了皮肤的修复与再生,为脂肪移植在非塑形领域提供了实验证据。
[Abstract]:Objective: the first part is to observe and evaluate the clinical effect of Nanofat transplantation in facial non-structural transplantation. The second part draws lessons from the animal model of injectable autogenous fat transplantation established by Tan Xin-et al to establish the animal experiment of Nanofat non-structural transplantation. Methods: part I, September 2014 to December 2014, A total of 10 female patients, including 1 case of skin aging in the cheek, 5 cases of skin aging around the eye, 2 cases of aging of the frontal skin, 2 cases of scar, and 2 cases of scar, were collected for the treatment of female patients with fat transplantation. After mechanical emulsification, Nanofat was retained and injected deep into the dermis of needle 27 until the skin color changed. In the second part, adipose tissue was obtained from the interscapular area or groin area of 6 New Zealand rabbits, and some fat particles were taken as experimental group after mechanical emulsification. Transplantation to the right ear of the rabbit and the control group to the left ear. The survival rate of fat and the effect on the skin of the two groups were observed on January and February. Results: the first part was effective in 9 cases. There were 7 cases of skin aging improved, 1 case of no serious complications, 7 cases of stable effect during the follow-up period, 3 cases of re-operation, 3 cases of the animal model of non-structural transplantation of fat injectable fat in rabbit ear, which was easy to observe and to measure in vivo. Conclusion: the first part of Nanofat graft is helpful for facial skin rejuvenation. The second part of Nanofat can destroy the tissue structure and lose cell activity of fat after mechanical emulsification, and affect the survival rate of fat transplantation. But it stimulates skin repair and regeneration, and provides experimental evidence for fat transplantation in the field of non-plastic.
