骨疏康胶囊对股骨粗隆间骨折PFNA术后的疗效观察

【摘要】：目的:研究骨疏康胶囊对股骨粗隆间骨折PFNA术后的临床疗效。方法:本研究纳入2015年9月至2016年3月厦门大学附属东南医院骨科住院病人符合纳入标准和排除标准的股骨粗隆间骨折的60名患者,按住院时间的先后顺序,采用随机数字表法,将患者分为治疗组(A组)和对照组(B组),每组各30例。术后第1天治疗组给予骨疏康胶囊口服,对照组给予钙尔奇D片口服,连续服用3个月。骨折均在牵引床上行PFNA内固定术。术前记录两组患者的一般资料。分别于术后第1天、术后3个月随访两组患者的血清钙(Ca)、磷(P)、骨碱性磷酸酶(BALP)、抗酒石酸酸性磷酸酶(TRACP5b)、桡骨远端骨密度(BMD)。记录术后第3、6、12个月髋关节Harris功能评分。结果:两组患者术前一般资料(性别、年龄、骨折分型及随访时间)比较无显著性差异(P0.05),两组病例均获得随访。治疗3个月后血清Ca、P变化情况,治疗组和对照组比较,差异具有统计学意义(P0.05),治疗组的血清Ca、P改善情况均优于对照组;治疗3个月后BALP、TRACP5b变化情况,治疗组和对照组比较,差异具有统计学意义(P0.05),治疗组的BALP、TRACP5b改善情况均优于对照组;治疗3个月后BMD变化情况,治疗组和对照组比较,差异具有统计学意义(P0.05),治疗组的BMD改善情况均优于对照组;治疗后3、6、12个月髋关节Harris功能评分情况比较,治疗组和对照组比较,差异具有统计学意义(P0.05),治疗组的髋关节功能改善情况较对照组显著。结论:骨疏康胶囊能改善股骨粗隆间骨折PFNA术后的骨代谢生化指标,提高骨密度,改善患髋的功能,既安全有效,而且经济实用,具有良好的临床应用价值。
[Abstract]:Objective: to study the clinical effect of Gushukang capsule on PFNA operation of intertrochanteric fracture of femur. Methods: from September 2015 to March 2016, the clinical effect of Gushukang capsule in the orthopedic department of Southeast Hospital affiliated to Xiamen University met the criteria of inclusion and exclusion. In 60 patients with an intertrochanteric fracture of the femur, According to the order of hospital stay, the patients were divided into treatment group (group A) and control group (group B, 30 cases in each group by random digital table method). The treatment group was given Gushukang capsule orally on the first day after operation, and the control group was given oral calcium D tablet. The fractures were treated with PFNA internal fixation on the traction bed. The general data of the two groups were recorded before operation. Three months after operation, the patients in the two groups were followed-up with serum calcium, phosphorous, bone alkaline phosphatase (BALP), tartrate acid phosphatase (tartrate acid phosphatase), bone mineral density (BMD) and distal radius bone density (BMD). The hip joint Harris functional scores were recorded at the 3rd and 12th month after operation. Results: the preoperative general data (sex, gender) of the two groups were as follows. There was no significant difference in age, fracture classification and follow-up time (P 0.05). The patients in both groups were followed up. After 3 months of treatment, the changes of serum CaP were compared between the treatment group and the control group. The difference was statistically significant (P 0.05), the improvement of serum CaP in the treatment group was better than that in the control group, the change of BALPnTRACP5b in the treatment group was higher than that in the control group after 3 months, the difference between the treatment group and the control group was statistically significant (P 0.05), and the improvement of the treatment group was better than that of the control group. After 3 months of treatment, the changes of BMD were significantly different between the treatment group and the control group (P 0.05). The improvement of BMD in the treatment group was better than that in the control group, and the Harris functional score of the hip joint in the treatment group was better than that in the control group 3 and 12 months after treatment, and the comparison between the treatment group and the control group. The improvement of hip function in the treatment group was more significant than that in the control group. Conclusion: Guxukang capsule can improve the biochemical indexes of bone metabolism, increase bone density and improve the function of the affected hip after PFNA operation for intertrochanteric fracture of femur. It is safe and effective, and economical and practical, and has good clinical application value.
