软骨下骨质疏松对骨关节炎病程发展的促进作用

【摘要】:目的:探讨软骨下骨质疏松对骨关节炎病程发展的促进作用。方法:选取3月龄雌性Wistar大鼠120只,所有大鼠均健康未孕,均接受2周的适应性饲养。依据体重将这些大鼠分为去势组(n=30)、去势对照组(n=30)、维甲酸组(n=30)、维甲酸对照组(n=30)四组,按组分笼饲养。制备大鼠模型,采集标本并制备关节标本病理切片,进行病理切片染色,然后对四组大鼠的左膝关节BMD、胫骨关节软骨厚度、病理学肉眼观察结果评分进行统计分析。结果:8周、12周时维甲酸组大鼠的左膝关节BMD均显著低于去势组(P0.05),胫骨关节软骨厚度均显著低于去势组(P0.05);8周时维甲酸组、去势组大鼠的胫骨关节软骨病理学肉眼观察结果评分0分、1分、2分、3分、4分比例之间的差异均不显著(P0.05),12周时维甲酸组大鼠的4分比例显著高于去势组(P0.05),但0分、1分、2分、3分比例之间的差异均不显著(P0.05)。结论:软骨下骨质疏松对骨关节炎病程发展有促进作用,随着软骨下骨质疏松的加重,骨关节炎不断加重。临床在对骨性关节炎进行治疗的过程中应该同时对骨质疏松进行治疗,只有这样才能收到较好的治疗效果。
[Abstract]:Objective: to investigate the effect of subchondral osteoporosis on the development of osteoarthritis. Methods: 120 3-month-old female Wistar rats were selected. All the rats were fed adaptively for 2 weeks. The rats were divided into four groups according to their body weight: castration group, castrated control group, retinoic acid group, retinoic acid control group and retinoic acid control group. The specimens were collected and the pathological sections of joint specimens were prepared and stained with pathological sections. Then the left knee joint BMD and tibial articular cartilage thickness of the four groups of rats were studied. Results the BMD of left knee in retinoic acid group was significantly lower than that of castrated group (P 0.05), and the thickness of tibial articular cartilage was significantly lower than that of ovariectomized group at 8 weeks. The pathological observation of tibial articular cartilage in ovariectomized rats showed no significant difference between 0, 1, 2, 3 and 4 points. The ratio of 4 points in the retinoic acid group was significantly higher than that in the castrated group at 12 weeks. Conclusion: subchondral osteoporosis can promote the development of osteoarthritis. With the exacerbation of subchondral osteoporosis, osteoarthritis is becoming more and more serious. In the course of treatment of osteoarthritis, osteoporosis should be treated at the same time, only in this way can better therapeutic effect be obtained.
