双联固定在后交叉韧带止点撕脱骨折中的应用研究

【摘要】：目的:探讨经膝关节后内侧小切口空心钉、带线锚钉双联固定治疗后交叉韧带胫骨撕脱骨折的手术效果。方法:选取重庆医科大学附属第二医院2010年03月2014年02月收治的17例后叉韧带(Posterior cruciate ligament,PCL)胫骨撕脱骨折患者,其中男性12例,女性5例,年龄19~56岁,平均38.7岁,右膝11例,左膝6例;致伤原因:交通伤8例,运动伤5例,摔倒4例,Meyers-Mc Keever分型,其中Ⅰ型2例,Ⅱ型8例,Ⅲ型7例;采用后内侧小切口空心钉联合带线锚钉固定,术后使用可调节支具固定,伤口拆线后支具保护下膝关节屈伸锻炼,4周左右部分负重锻炼,6~8周逐渐拆除支具至正常负重。参照Lysholm膝关节评分系统评价疗效。结果:17例患者术后均得到随访,随访时间6~12月。所有患者伤口均一期愈合,无伤口感染、血管神经损伤、深静脉血栓(deep vein thrombosis,DVT)形成等严重并发症,术后3~6月,平均4.2月骨性愈合,无带线锚钉及空心螺钉松动,Lachman实验阴性,Lysholm评分显示优秀15例,良2例。结论:空心钉和带线锚钉双联固定治疗PCL胫骨撕脱骨折学习曲线短,操作容易,术后能更早开始功能锻炼,获得更好的功能,效果满意。
[Abstract]:Objective: to study the transarticular posterior medial small incision hollow nail. Surgical results of tibial avulsion fracture of posterior cruciate ligament treated with wire anchor nail. Methods: 17 cases of posterior cruciate ligament tibial avulsion fracture were treated in the second affiliated Hospital of Chongqing Medical University on March 2010 and February 2014. Among them, 12 cases were male, 5 cases were female, the age was 1956 years old (mean 38.7 years), 11 cases were right knee and 6 cases were left knee, the causes of injury were traffic injury (8 cases), sports injury (5 cases), fall (4 cases) according to Meyers-Mc Keever classification, including type 鈪，
