氨甲环酸减少双侧全膝关节置换术失血量的有效性及安全性研究

【摘要】：目的:探讨氨甲环酸对双侧全膝关节置换术患者围术期出血量、血红蛋白变化的影响及其安全性的研究。方法:选取本院69例需进行双侧全膝关节置换术的患者,随机分为2组,实验组给予氨甲环酸1g溶于100ml 5%葡萄糖溶液中静脉滴注,对照组给予5%葡萄糖溶液100ml静脉滴注。比较两组的总失血量、术中失血量、隐性失血量、术后引流量、输血率、血红蛋白、D-二聚体、凝血酶原时间和活化的部分凝血酶时间等指标的动态变化情况,并研究两组血栓事件的发生率。结果:实验组的失血量明显低于对照组(t=2.194~2.908,P0.05),输血率也明显低于对照组(χ2=5.219,P=0.022)。比较术后第3天和第5天的血红蛋白量,实验组明显高于对照组(t=-3.481~2.319,P0.05)。术前术后两组观察者的凝血酶原时间、活化的部分凝血酶时间无明显差异(P0.05)。结论:氨甲环酸能显著减少双侧全膝关节置换术患者的围术期失血量,降低对输血的需求,减小血红蛋白的动态变化,同时不增加血栓形成的风险。
[Abstract]:Objective: to study the effect of carbamoic acid on perioperative bleeding, hemoglobin change and safety of bilateral total knee arthroplasty. Methods: 69 patients with bilateral total knee arthroplasty in our hospital were selected. Two groups were randomly divided into two groups: the experimental group was given 1 g of methacylic acid dissolved in 100ml 5% glucose solution, and the control group was given 5% glucose solution 100ml intravenous drip. The total blood loss, intraoperative blood loss, recessive blood loss and postoperative drainage were compared between the two groups. The dynamic changes of blood transfusion rate, hemoglobin D-dimer, prothrombin time and activated partial thrombin time, Results: the blood loss in the experimental group was significantly lower than that in the control group, and the blood transfusion rate was significantly lower than that in the control group. There was no significant difference in prothrombin time and activated partial thrombin time between the two groups before and after operation. Conclusion: the perioperative blood loss of bilateral total knee arthroplasty patients can be significantly reduced by methacylic acid. Reduce the need for blood transfusions, reduce the dynamic changes in hemoglobin, without increasing the risk of thrombosis.
