股骨髁间窝宽度和前交叉韧带胫骨端左右径相关性及其与前交叉韧带损伤的关系

【摘要】：目的:探讨股骨髁间窝宽度与前交叉韧带(ACL)胫骨端左右径之间的相关性及其与ACL损伤的关系,为ACL重建术提供解剖依据。方法:选取在本院接受膝关节镜检的患者,ACL损伤组17人,ACL完整组20人,搜集其性别、年龄、是否为运动员、受伤至手术时间等信息。术中在关节镜下测量ACL胫骨端左右径和髁间窝上1/3、下1/3及基底部宽度。分别比较ACL损伤组和ACL完整组、男性和女性患者、ACL损伤组男性和ACL完整组男性、ACL完整组女性和ACL完整组男性患者之间测量指标有无统计学差异,并分别分析髁间窝宽度与ACL胫骨端左右径之间有无相关性。结果:(1)各组别患者的髁间窝宽度、ACL胫骨端左右径绝对值比较均无统计学差异。(2)ACL损伤组患者髁间窝宽度与ACL胫骨端左右径不具有相关性,而ACL完整组患者具有中等程度相关性;单独分析男性患者,发现ACL完整组男性患者患者髁间窝宽度与ACL胫骨端左右径具有高度相关性,而ACL损伤组男性患者不具有相关性,提示髁间窝宽度与ACL胫骨端左右径不匹配可能是非接触性ACL损伤发生的危险因素之一。(3)全体男性患者的髁间窝宽度与ACL胫骨端左右径具有中等程度相关性,而全体女性患者则不具有相关性;去掉ACL损伤患者,发现ACL完整组男性患者髁间窝宽度与ACL胫骨端左右径具有高度相关性,而ACL完整组女性患者则不具有相关性,提示髁间窝宽度与ACL胫骨端左右径不匹配可能是女性非接触性ACL损伤发生率高于男性的原因之一。结论:ACL胫骨端左右径相对于髁间窝宽度太宽或太窄可能是ACL损伤的危险因素,也可能是女性ACL损伤发生率高于男性的原因之一。
[Abstract]:Objective: to investigate the correlation between the width of femoral intercondylar fossa and the tibial diameters of anterior cruciate ligament (ACL) and its relationship with ACL injury. Methods: a total of 17 patients with ACL injury in our hospital underwent arthroscopy, and 20 patients in the intact group were selected to collect their sex, age, and whether they were athletes, and to provide anatomical basis for the reconstruction of ACL. The right and left diameters of the tibial end of ACL and the width of the upper 1 / 3 of the intercondylar fossa, the lower 1/3 and the base base width were measured under arthroscopy. The ACL injury group and the ACL intact group were compared, respectively. Whether there was statistical difference between male and female patients with ACL-injury and ACL intact group with ACL-intact group and ACL group. The correlation between the width of the intercondylar fossa and the left and right diameters of the tibial end of ACL was analyzed respectively. Results there was no significant difference in the width of the intercondylar fossa and the left and right diameters of the tibia between the two groups. There was no correlation with the left and right diameters of the tibial end of ACL. It was found that the width of intercondylar fossa was highly correlated with the left and right diameters of ACL tibia in ACL intact group, but that in ACL injury group, there was no correlation between the width of intercondylar fossa and the left and right diameters of tibia in ACL intact group. It is suggested that the mismatch between the width of the intercondylar fossa and the left and right diameters of the tibial end of ACL may be one of the risk factors for non-contact ACL injury.) the width of the intercondylar fossa in all male patients is moderately correlated with the left and right diameters of the tibial end of ACL. The width of intercondylar fossa was highly correlated with the left and right diameters of ACL tibia in the ACL intact group, but not in the ACL intact group. It is suggested that the mismatch between the width of the intercondylar fossa and the left and right diameters of the tibial end of ACL may be one of the reasons for the higher incidence of non-contact ACL injury in women than in the male. Conclusion the width of the left and right tibial ends of the tibial ends of ACL is too wide or too narrow relative to the width of the intercondylar fossa. It is also possible that the incidence of ACL damage in women is higher than that in men.
