椎体成形骨填充材料的研究与现状

【摘要】:背景:椎体成形是治疗骨质疏松性椎体压缩骨折安全、有效的微创治疗技术。目的:综述椎体成形中骨充材料的生物力学性能、骨强度维持状态、骨降解吸收及诱导成骨等方面的研究进展。方法:第一作者应用计算机检索中国知网、PubMed、Medline数据库2005年1月至2016年5月的相关文章,英文检索词为"Bone cement;Bone filling materials;Percutaneous vertebroplasty",中文检索词为"骨水泥;骨填充材料;椎体成形术"。结果与结论:聚甲基丙烯酸甲酯骨水泥不是骨质疏松性椎体压缩骨折椎体填充的理想材料。磷酸钙骨水泥和硫酸钙骨水泥可替代传统聚甲基丙烯酸甲酯骨水泥,但磷酸钙骨水泥和硫酸钙骨水泥也存在一些问题,如显影效果不理想、生物降解速度难以控制、远期效果缺乏进一步循证医学的支持等。复合骨水泥兼顾了多种骨水泥的优点,是良好的骨修复材料,但进入临床时间短,还需大量的临床实践证明,是目前研究的主要方向。
[Abstract]:Background: vertebroplasty is a safe and effective minimally invasive technique for the treatment of osteoporotic vertebral compression fractures. Objective: to review the biomechanical properties and bone strength maintenance of bone filling materials in vertebroplasty. Advances in bone degradation and osteogenesis methods: the first author used a computer to search the relevant articles in the PubMedline database of China from January 2005 to May 2016. The English key word is "Bone cemented bone filling material vertebroplasty", the Chinese key word is "bone cement, bone filling material material", and the Chinese key word is "bone cement; bone filling material". Results & conclusion: polymethyl methacrylate bone cement is not an ideal material for vertebral body filling in osteoporotic vertebral compression fracture. Calcium phosphate cement and calcium sulfate cement can replace traditional polymethyl methacrylate cement. However, there are some problems in calcium phosphate cement and calcium sulfate cement, for example, the development effect is not ideal, the biodegradation rate is difficult to control, the long-term effect is short of the support of further evidence-based medicine and so on. The composite bone cement takes into account the advantages of many kinds of bone cement. It is a good bone repair material, but the clinical time is short and a lot of clinical practice is needed. It is the main research direction at present.
