双源CT对前交叉韧带单束重建术后早期效果的评价

【摘要】：目的：探讨双源CT评价膝关节镜下前交叉韧带单束重建术后早期效果及其临床应用价值。方法：对2014年7月——2015年2月我院收治的50例(其中男34例,女16例)关节镜下前交叉韧带单束重建患者于术后第二天行双源CT扫描。使用CT图像后处理工作站三维重建膝关节股骨外髁、胫骨平台三维模型,重建ACL移植物,再现ACL自然印记及骨道位置,测量骨道中心点相对位置,观察移植物走行。结果：胫骨骨隧道中心点位置为：矢状面上,骨隧道中心点相对位置为48%；冠状面上,骨隧道中心点相对位置为51%。股骨骨隧道中心点位置为：由近到远股骨髁高度的20%,在Blumensaat线水平由后向前方向股骨外髁长度的25%。ACL移植物走向良好并与髁间窝无撞击的有48例,发生撞击的有2例。结论：双源CT三维重建能够清晰地显示ACL股骨、胫骨骨隧道位置,再现ACL移植物的走向,可用于临床评估术后骨隧道与移植物位置关系,对临床ACL重建术后效果评估具有很好的指导意义。
[Abstract]:Objective: to evaluate the early effect of single-bundle reconstruction of anterior cruciate ligament (ACL) under knee arthroscopy by dual-source CT and its clinical value. Methods: from July 2014 to February 2015, 50 cases (34 males) were treated in our hospital. Sixteen female patients underwent dual-source CT scanning on the second day after arthroscopic reconstruction of the anterior cruciate ligament. Three-dimensional reconstruction of the lateral femoral condyle and tibial plateau of the knee joint was performed with a post-processing workstation for reconstruction of ACL graft. The ACL natural imprint and the position of bone canal were reproduced, the relative position of the center point of bone passage was measured, and the walking of the graft was observed. The relative position of the center point of the bone tunnel is 51. The central point of the femoral bone tunnel is 20 from the proximal to the distal femoral condyle. At the level of Blumensaat line, 25% of the lateral femoral condyle length from the posterior to the front of the femur moves well, and there is no impact with the intercondylar fossa in 48 cases. Conclusion: the position of bone tunnel of ACL femur and tibia can be clearly displayed by 3D reconstruction of dual source CT, and the direction of ACL graft can be reappeared, which can be used to evaluate the relationship between bone tunnel and graft position after operation. It is of great significance to evaluate the effect of ACL reconstruction.
