下腰椎间盘突出与髂腰韧带力学强度的影像学分析

【摘要】：目的探讨下位腰椎间盘突出与髂腰韧带力学强度的关系。方法收入我院脊柱外科2012年至2014年收治的130例下位腰椎间盘突出症患者其中L4/5节段73例,L5/S1节段57例。正常组为我科没有腰痛病史的医护以及进修医师45例。在腰椎正位片上测量并计算出各病例第5腰椎横突的长度,宽度,椎弓根间的距离,横突宽度与椎弓根内间距比值(RT值),横突长度与椎弓根内间距比值(RL值),对所得数值进行统计学分析。结果正常组、椎间盘突出组在年龄、性别差异无显著性意义。腰椎间盘突出症组的椎弓根间距离为29.6mm显著小于正常组30.8mm(P=0.0032),横突长度的左右差为1.84正常组为1.36,左右比值椎间盘突出组1.054正常组为1.040,表示椎间盘突出组比正常组有增大倾向。同时,椎间盘突出组RL左右差为6.25,正常组为4.51差异具有统计学意义(P=0.0408)。按照腰椎间盘突出节段分为L4/5和L5/S1组。测量L5横突宽度,L4/5组:19.9mm,L5/S1组:18.3mm由此计算的RT值L4/5:66.7,L5/S1:62.9,L4/5组比L5/S1组的L5横突宽度以及RT均有显著统计学差异。结论髂腰韧带力学强度与椎间盘突出相关,并影响突出节段。
[Abstract]:Objective to investigate the relationship between lumbar disc herniation and the mechanical strength of iliolumbar ligament. Methods 130 patients with L4 / 5 segment L5 / S1 segment were admitted to our hospital from 2012 to 2014. The normal group consisted of 45 medical and advanced physicians who had no history of low back pain. The length of the transverse process of the fifth lumbar vertebrae was measured and calculated on the lumbar vertebrae positive radiographs. The width, the distance between pedicles, the ratio of transverse process width to pedicle distance, the ratio of transverse process length to pedicle distance, and the ratio of transverse process length to pedicle distance were analyzed statistically. Results in the normal group, the intervertebral disc herniation group was in age. The distance between pedicle of lumbar intervertebral disc herniation group (29.6mm) was significantly lower than that of normal group (30.8 mm), the left and right difference of transverse process length was 1.36 in normal group, and the ratio of intervertebral disc herniation was 1.040 in 1.054 normal group. The disc herniation group had a tendency to increase compared with the normal group. At the same time, The difference between left and right RL was 6.25 in disc herniation group and 4.51 in normal group. According to the segment of lumbar disc herniation, L4 / 5 and L5/S1 groups were divided into L4 / 5 and L5/S1 groups. The RT value of L4 / 5: 66.7% L5 / S1: 62.9% L4 / 5 was calculated by measuring the width of L4 / 5 transverse process in L4 / 5 group as compared with that of L5 / 5 group in L5/S1 group. Conclusion the mechanical strength of iliolumbar ligament is associated with disc herniation. And influence protruding segment.
