填充网袋椎体成形术治疗胸腰椎骨质疏松性骨折

【摘要】：[目的]观察骨填充网袋椎体成形术治疗老年胸腰椎骨质疏松性骨折临床疗效观察。[方法]老年胸腰椎骨质疏松性骨折23例共31椎体,采用骨填充网袋椎体成形术治疗,观察VAS评分、伤椎体高度压缩恢复率及Cobb角变化,随访至术后6个月。[结果]VAS评分由术前的(7.60±1.00)分,降至术后7 d的(2.70±0.70)分;术前伤椎前缘高度为(45.30±11.60)%,椎体中部高度为(47.80±12.50)%,术后7 d分别提高至(72.10±13.50)%、(73.70±15.40)%;术前椎后凸畸形角(19.30±6.20)°,术后7 d降低至(14.30±3.50)°;术前VAS评分、伤椎前缘高度、椎体中部高度、椎体后凸畸形角,与术后7 d,1、6个月比较差异有统计学意义(P0.01)。发生骨水泥渗漏1例(4.3%)。[结论]骨填充网袋结合椎体成形术治疗老年胸腰椎骨质疏松性骨折,能有效恢复伤椎高度,快速止痛,且骨水泥渗漏发生率低,安全可靠。
[Abstract]:[objective] to observe the clinical effect of bone-filled pouch vertebroplasty in the treatment of senile thoracolumbar osteoporotic fracture. [methods] Twenty-three cases of senile thoracolumbar osteoporotic fracture were treated with bone-filled pouch vertebroplasty. VAS score, recovery rate of high compression of injured vertebral body and change of Cobb angle were observed and followed up to 6 months after operation. [results] VAS score decreased from 7.60 卤1.00 before operation to 2.70 卤0.70 at 7 days after operation. The anterior height of the injured vertebrae was 45.30 卤11.60 掳, the height of the middle part of the vertebral body was 47.80 卤12.50, which was increased to 72.10 卤13.50 卤73.70 卤15.40 掳on the 7th day after operation, the angle of the kyphosis was 19.30 卤6.20 掳before operation and decreased to 14.30 卤3.50 掳on the 7th day after operation; the preoperative VAS score showed the height of the anterior edge of the injured vertebra, the height of the middle vertebral body, the angle of kyphosis deformity. There was a significant difference between 7 days and 6 months after operation (P 0.01). Bone cement leakage occurred in 1 case (4.3%). [conclusion] the treatment of osteoporotic fracture of thoracolumbar vertebrae with bone pouch combined with vertebroplasty can effectively recover the height of injured vertebrae and relieve pain quickly. And the incidence of bone cement leakage is low, safe and reliable.
