氨甲环酸联合自体血液回输器对TKA术后出血量与DVT的临床研究

【摘要】：目的:探讨氨甲环酸联合自体血液回输器应用于全膝关节置换术的临床疗效。方法:选择山东省中医院2012年5月至2014年12月期间行TKA的患者60例,采用随机单盲对照分组的方法,分为试验组(氨甲环酸自体血液回输器联合应用)与对照组(只应用自体血液回输器),记录相关数据,计算术后隐性失血量与总失血量,统计分析。结果:试验组自体血回输量、引流量、术后3d隐性失血量、术后3d再次输血量、再次输血例数与异体输血量较对照组减少,试验组术后3d红细胞计数、血红蛋白、红细胞压积较对照组数值高,上述均有统计学差异(P0.05)。两组均未见深静脉血栓形成,其他并发症无统计学差异。结论:氨甲环酸联合自体血液回输器能减少出血量与异体输血量,不增加深静脉血栓与其他并发症的风险。
[Abstract]:Objective: to investigate the clinical effect of total knee arthroplasty (TKR) treated with carbamoic acid combined with autologous blood transfusion. Methods: 60 patients with TKA from May 2012 to December 2014 in Shandong traditional Chinese Medicine Hospital were selected. A randomized single-blind control group was divided into two groups: the experimental group (combined with the autogenous blood transfusion) and the control group (using the autologous blood transfusion device only) to record the relevant data, and to calculate the recessive blood loss and the total blood loss after operation. Results: the amount of autologous blood transfusion, drainage volume, recessive blood loss on the 3rd day after operation, re-transfusion volume on the 3rd day after operation, the number of cases of re-transfusion and the volume of allogeneic blood transfusion in the test group were lower than those in the control group, and the red blood cell count and hemoglobin in the test group on the 3rd day after operation. The hematocrit was higher than that of the control group (P 0.05). There was no significant difference between the two groups in deep venous thrombosis and no significant difference in other complications. Conclusion: the amount of blood loss and the amount of allogeneic blood transfusion can be reduced by the combination of carbamate and autologous blood transfusion. There is no increased risk of deep venous thrombosis and other complications.
