负压封闭引流术在软组织创面中的临床应用

【摘要】：目的分析负压封闭引流术对软组织创面血管、神经生成的影响,探讨负压封闭引流术在软组织创面中的临床效果。方法选取70例软组织创面患者作为研究对象,根据入院先后顺序进行分组;观察组35例,采取负压封闭引流术治疗;对照组35例,采取传统换药治疗;对比两组患者的首次植皮存活率、细菌转阴率、创面修复时间、手术次数、愈合时间,根据治疗前、治疗后7、14d的微血管数量、神经生长因子(NGF)阳性细胞数,综合评价患者的神经生成情况。结果观察组首次植皮存活率、细菌转阴率均大于对照组,创面修复时间、愈合时间均短于对照组,手术次数少于对照组,差异有统计学意义(P0.05);治疗前,两组患者的微血管数量、NGF阳性细胞数差异无统计学意义(P0.05);治疗后7、14d,观察组患者的微血管数量、NGF阳性细胞数均较治疗前明显增多,且与对照组比较,差异有统计学意义(P0.05)。结论负压封闭引流术可促进软组织创面血管、神经生成,对于促进软组织创面愈合,改善预后均具有积极作用。
[Abstract]:Objective to analyze the effect of negative pressure blocking drainage on the formation of blood vessels and nerves in soft tissue wound and to explore the clinical effect of negative pressure blocking drainage on soft tissue wound.Methods 70 patients with soft tissue wounds were divided into three groups according to the order of admission, the observation group (35 cases) were treated with negative pressure blocking drainage, the control group (35 cases) were treated with traditional dressing change.The survival rate of the first skin graft, the rate of bacterial negative conversion, the time of wound repair, the times of operation, the time of healing, the number of microvessels and the number of NGF positive cells before and after treatment were compared between the two groups.Comprehensive evaluation of neurogenesis in patients.Results the survival rate of the first skin graft and the negative rate of bacteria in the observation group were all higher than those in the control group. The wound repair time and healing time were shorter in the observation group than in the control group, and the number of operations was less than that in the control group, the difference was statistically significant (P 0.05).There was no significant difference in the number of NGF positive cells between the two groups (P 0.05), but at 714 days after treatment, the number of NGF positive cells in the observation group was significantly higher than that before treatment, and the difference was statistically significant compared with the control group (P 0.05).Conclusion negative pressure blocking drainage can promote the formation of blood vessels and nerves in soft tissue wound, and play a positive role in promoting the healing of soft tissue wound and improving the prognosis.
