交锁髓内钉与锁定加压钢板治疗胫骨骨折不愈合的疗效比较分析

【摘要】：目的:初步比较交锁髓内钉与锁定加压钢板治疗胫骨骨折不愈合的临床疗效。方法:选择吕梁市人民医院骨科2012年1月至2014年2月收治的胫骨骨折不愈合患者56例,其中男性34例,女性22例,年龄15-65岁,平均36岁。其病因:砸伤20例,高处坠落受伤20例,交通受伤16例。根据骨不愈合的分型方法[3]:骨端萎缩型14例,骨端正常型18例,骨端肥大型24例;第一次手术时间为伤后1-10天,平均4天。第一次治疗方案:保守治疗10例,手术治疗46例,其中用髓内钉固定15例,钢板内固定18例,外固定架固定13例。将56例患者随机分为交锁髓内钉组(30例)和锁定加压钢板组(26例);交锁髓内钉组30例,男性17例,女性13例;锁定加压钢板组26例,男性17例,女性9例,观察两组的临床疗效。结果:交锁髓内钉组患者的平均手术时间较锁定加压钢板组短(P0.05),但是两组患者的骨折平均愈合时间、疗效比较差异无统计学意义(P0.05)。结论:交锁髓内钉和锁定加压钢板均是治疗胫骨骨折不愈合的有效方法。
[Abstract]:Objective: to compare the clinical effect of interlocking intramedullary nail and locking compression plate in the treatment of tibial fracture nonunion.Methods: from January 2012 to February 2014, 56 patients with nonunion of tibial fractures were selected from orthopedic department of Luliang people's Hospital, including 34 males and 22 females, aged 15-65 years with an average of 36 years.The etiology: 20 cases of smashing injury, 20 cases of falling injury and 16 cases of traffic injury.According to the classification method of bone nonunion, 14 cases of bone end atrophy type, 18 cases of bone end normal type, 24 cases of large bone end fat, the first operation time is 1-10 days after injury, average 4 days.The first treatment plan: conservative treatment 10 cases, surgical treatment 46 cases, including 15 cases with intramedullary nail fixation, 18 cases with plate fixation, 13 cases with external fixation frame.56 patients were randomly divided into interlocking intramedullary nail group (n = 30) and locking compression plate group (n = 26); interlocking intramedullary nail group (n = 30, male 17 and female, n = 13); locking compression plate group, n = 26, male = 17, female = 9,To observe the clinical effect of the two groups.Results: the average operative time of interlocking intramedullary nail group was shorter than that of locking compression plate group (P 0.05), but there was no significant difference in the average healing time and curative effect between the two groups.Conclusion: interlocking intramedullary nail and locking compression plate are effective methods in the treatment of tibial nonunion.
