脊柱骨盆矢状面形态与腰椎间盘退变的相关性

【摘要】：目的:探讨腰椎间盘退变的位置、范围、程度、与脊柱骨盆矢状位参数的关系。方法:回顾性分析2013年1月至2015年12月间76例脊柱外科腰椎间盘退变疾病(腰椎间盘突出症、退变性腰椎不稳症)患者临床资料。统计:1、患者一般资料:姓名、性别、年龄、身高、体重;2、脊柱全长片上测量矢状位参数(TK、LL、SS、PT、PI、SVA)3、腰椎间盘退变的位置、范围、程度。对各参数进行统计分析,按性别、PI的大小进行分组分析,同时腰椎间盘退变的位置、范围、程度与脊柱骨盆矢状位参数行相关性分析。结果:(1)男性患者和女性患者平均年龄分别是58.86岁、53.28岁(P㩳0.05),退变的位置值、范围值、程度值分别是3.74±0.78、3.92±1.07(P㧐0.05)、2.41±1.36、1.81±1.01(P㩳0.05)、3.73±0.73、3.38±0.54(P㩳0.05)。(2)年龄与TK(r=0.376,P㩳0.01)、年龄与退变位置(r=-0.358,P㩳0.01)、年龄与退变范围(r=0.329,P㩳0.01)、年龄与退变程度(r=0.452,P㩳0.01);TK与LL(r=0.531,P㩳0.01);LL和PI(r=0.409,P㩳0.01);LL与SS(r=0.827,P㩳0.01)、PI与SS(r=0.532,P㩳0.01)、PI与PT(r=0.711,P㩳0.01);PI与退变性位置(r=-0.257,P㩳0.05);SVA与退变的范围(r=0.304,P㩳0.01)、SVA与退变程度(r=0.353,P㩳0.01)。(3)按PI大小分组后,PI≤50°组患者位置值为4.14±0.64,PI50°组患者位置值为3.57±1.08(P㩳0.05)。结论:1、随退变的发生,脊柱骨盆矢状面不断代偿来维持站立时的重心,减少能量消耗。2、腰椎间盘退变的位置、范围、程度与年龄密切相关。3、腰椎间盘退变的位置多发生于下腰椎,更具体位置受脊柱骨盆矢状面形态的影响,小PI人群倾向于L4/5、L5/S1椎间盘的退变,大PI人群更倾向于L3/4,L4/5椎间盘的退变。
