纤维蛋白胶对全膝关节置换术后疗效影响的Meta分析

【摘要】：目的:探讨纤维蛋白胶(fibrin glue,FG)在全膝关节置换术(total knee arthroplasty,TKA)中应用的有效性和安全性。方法:通过检索Pub Med、Embase、Cochrane Library、CNKI、CBM、万方等数据库,同时手工检索图书馆馆藏期刊,由2位研究者按照纳入和排除标准独立筛选高质量的随机对照试验(randomized controlled trials,RCTs)文献,采用Rev Man 5.2软件对提取的数据进行统计学分析。结果:共纳入11个RCTs,共973个TKA患者。Meta分析结果显示:与对照组相比,纤维蛋白胶可以减少TKA术后引流(MD=-227.36,95%CI=-359.47~-95.25,P=0.001);血红蛋白丢失量(MD=-0.63,95%CI=-0.79~-0.46,P=0.000);总失血量(MD=-235.59,95%CI=-453.31~-17.86,P=0.030);输血率(OR=0.44,95%CI=0.31~0.61,P=0.000);且不增加术后并发症的发生(OR=0.94,95%CI=0.61~1.45,P=0.790)。结论:FG可以有效减少TKA术后引流量,总失血量,血红蛋白丢失量,输血率但不增加术后并发症的发生。
