全髋关节置换术后伤口延迟愈合与高血压的相关性研究

【摘要】:[目的]探讨高血压对全髋关节置换术后伤口持续渗出和延迟愈合的影响。[方法]回顾性分析2013年1月~2016年4月间解放军第81医院骨科收治的167例老年股骨颈骨折患者资料,男91例,女76例,年龄65~90岁,平均74.56岁。所有患者均接受全髋关节置换术治疗;根据《中国高血压防治指南2010》中的高血压诊断标准分为高血压组和非高血压组,主要比较两组术中失血量、术后失血量、拆线时间、伤口持续渗出时间、伤口裂开和伤口延迟愈合发生率是否有差异,分析高血压与伤口延迟愈合的关系。两组间数据的比较采用独立样本t检验。[结果]高血压组患者平均收缩压为151.86 mmHg(1 mmHg=0.133 kPa),非高血压组为122.93 mmHg,差异有统计学意义(P0.05);两组患者平均年龄、性别、体质量指数(BMI)、术前合并糖尿病情况、致伤机制、术前住院时间、术中及术后失血量差异均无统计学意义(P0.05);高血压患者伤口持续渗出时间为3.82 d,非高血压患者为1.95 d,差异有统计学意义(P0.01);高血压组伤口拆线时间为15.35 d,明显高于非高血压组的13.42 d(P0.01);高血压组切口延迟愈合发生率明显高于非高血压组(31.43%VS 14.52%),差异有统计学意义(P0.01)。5例高血压患者发生伤口裂开,非高血压患者未发生伤口裂开,因例数太少,未进行统计学分析。[结论]高血压患者全髋关节置换术后伤口持续渗出和伤口延迟愈合发生率的风险明显增高,高血压是切口延迟愈合的重要影响因素之一。
[Abstract]:[Objective] to investigate the effect of total hip arthroplasty] postoperative wound sustained exudation and delayed healing of hypertension. Methods a retrospective analysis of 167 cases of senile femoral neck in January 2013 ~2016 year in April admitted to the Department of orthopedics, eighty-first Hospital of PLA fracture patients, male 91 cases, female 76 cases, age 65~90 years old, average 74.56 years old. For 122.93 mmHg, the difference was statistically significant (P0.05); the average age of the two groups of patients, gender, body mass index (BMI), diabetes mellitus before operation, mechanism of injury, hospitalization time of preoperative, intraoperative and postoperative blood loss showed no significant difference (P0.05); high blood pressure in patients with persistent wound from time 3.82 D, non hypertensive patients was 1.95 D, the difference was statistically significant (P0.01); hypertension group suture removal time was 15.35 D, significantly higher than non hypertension group 13.42 D (P0.01); hypertension group the incidence of delayed wound healing rate was significantly higher in non hypertension group (31.43%VS 14.52%), the difference was statistically significant (P0.01).5 cases of hypertensive patients with non hypertensive patients without wound dehiscence, wound dehiscence occurred, because the number of cases is too small, not for statistical analysis. Conclusion: hypertension patients after total hip replacement wound sustained exudates and delayed wound healing rate The risk was significantly higher, and hypertension was one of the most important factors for the delayed healing of the incision.
