开窗式髓核摘除术结合独活寄生汤加减治疗腰椎间盘突出症疗效观察

【摘要】：目的:观察开窗式髓核摘除术结合独活寄生汤加减治疗腰椎间盘突出症的临床疗效。材料与方法:将辽宁中医药大学附属医院骨一科2014年3月至2016年12月间接受开窗式髓核摘除术的42例病例,随机分为中药组与对照组,中药组采取手术结合中药汤剂治疗,对照组采取单纯手术治疗,通过对总体病例术前、术后2周VAS评分行统计学分析,判定开窗式髓核摘除术的总体疗效,对两组病例术后3个月及末次随访ODI评分行组间统计学分析,判定加减独活寄生汤对开窗式髓核摘除术术后疗效的影响,进而评价开窗式髓核摘除术结合独活寄生汤加减治疗腰椎间盘突出症的临床疗效。结果:42例病例,术后平均随访21.2个月,术前VAS评分(8.9±0.51),术后2周VAS评分(2.8±0.53),自身前后比较P㩳0.05,具有统计学意义;术后3个月,中药组ODI评分(0.19±0.04),对照组ODI评分(0.26±0.06),组间比较P㩳0.05,具有统计学意义;末次随访,中药组ODI评分(0.09±0.03),对照组ODI评分(0.13±0.05),组间比较P㩳0.05,具有统计学意义。结论:1.开窗式髓核摘除术治疗腰椎间盘突出症疗效确实;2.术后结合独活寄生汤加减治疗,短期内可有效改善全身症状,提高生活质量,但长期疗效有待随访观察。
[Abstract]:Objective: To observe the clinical effect of opening window type nucleus pulposus removal combined with single live parasitic soup in the treatment of protrusion of lumbar intervertebral disc. Materials and methods: 42 cases of open window nucleus pulposectomy from March 2014 to December 2016 of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine were randomly divided into Chinese Medicine group and control group, and the Chinese medicine group was operated by the Chinese medicine group. Combined with traditional Chinese medicine decoction treatment, the control group was treated with simple operation. By statistical analysis of the total cases before operation and 2 weeks after operation VAS, the total curative effect of the open window type nucleus pulposectomy was determined. The statistical scores of the two groups after 3 months and the last follow-up of the two groups were analyzed, and the extirpation of the open window type nucleus pulposus was determined by the addition and subtraction of the single live parasitic soup. The effect of postoperative curative effect, and then to evaluate the clinical effect of open window type nucleus pulposus removal combined with single live parasitic soup in the treatment of lumbar disc herniation. Results: 42 cases were followed up for 21.2 months, the preoperative VAS score (8.9 + 0.51), and 2 weeks VAS score (2.8 + 0.53) after operation, P? 0.05, and 3 months after the operation. The ODI score of the Chinese medicine group (0.19 + 0.04), the control group ODI score (0.26 + 0.06), P? 0.05, was statistically significant; the last follow-up, the traditional Chinese medicine group ODI score (0.09 + 0.03), the control group ODI score (0.13 + 0.05), the group was P? 0.05, with statistical significance. Conclusion: 1. open window nucleus extraction for the treatment of lumbar disc herniation is effective; 2.; 2.; 2. The combination of the modified "angelica and mistletoe Decoction" can effectively improve systemic symptoms and improve the quality of life in the short term, but the long-term effect remains to be followed up.
