【摘要】：研究目的明确老年髋部骨折后急性肺损伤是否发生,血液中炎症因子(IL-1β, IL-6, IL-10,TNF-α)、mtNDA的浓度的表达变化,炎症因子的表达变化与mtDNA之间是否相关。研究方法采用24只雄性SD老年大鼠作为实验对象,随机分成老年髋部骨折组(20只)、空白对照组(4只),分别于处理后4h、8h、24h、48h、72h抽血,行炎症因子IL-1β、IL-6、IL-10、TNF-α、mtDNA的定量检测,处死后的动物,取肺部组织,测肺通透系数、水通道蛋白浓度、肺组织干湿重比以及HE染色,以探讨mtDNA与炎症因子间的相互关系,为老年髋部骨折并发肺损伤的治疗提供新的理论依据。结果1. 老年髋部骨折大鼠外周血IL-1β、IL-6、TNF-α水平随着时间增加逐渐升高,到24小时达到最高值,随后逐渐下降,在各个时间点都明显高于对照组,IL-10则是先下降到24h达到最低值,而后逐渐升高。外周血中的mtDNA在髋部骨折创伤发生后即升高,且在24h达到高值,随后下降,通过相关分析发现,mtDNA的与炎症因子IL-6的水平成正相关,与炎性因子IL-10成负相关。2. 与对照组正常老年大鼠比较,老年髋部骨折组的肺组织干湿重比、肺通透指数明显升高,且差异具有统计学意义(P(0.05),肺组织行western blotting结果显示AQP-1显著低于对照组(P0.05),肺组织冰冻切片后行HE染色可见肺组织光镜下部分组织细胞肿胀、细胞质淡染,细胞体积变大,肺泡间隔增宽,肺泡腔内有较多红细胞和大量中性粒细胞浸润,肺病理损伤积分显著高于正常对照组老鼠。结论1 髋部骨折老年大鼠外周血中的IL-1β、IL-6、TNF-α的含量随着时间推移逐渐增加,并在24h达到高峰,且在随后出现逐渐下降的趋势,IL-10的含量随时间推移先是减少在24h达到最低值后又逐渐增加。2 老年大鼠遭遇髋部骨折后可以引起急性肺损伤,肺组织干湿重比增加,肺通透系数增加,AQP-1的表达水平降低,病理表现为肺组织水肿,肺泡内出血、炎性细胞浸润、肺组织机构发生紊乱。3 外周血mtDNA在髋部骨折的老年大鼠体内升高,并且与炎症反应的发生呈正相关性。
[Abstract]:Objective to determine whether acute lung injury occurs after hip fracture in the elderly, and whether the expression of IL-1 尾, IL-6, IL-10 TNF- 伪 mtNDA in blood is correlated with mtDNA. Methods Twenty-four male Sprague-Dawley (SD) aged rats were randomly divided into elderly hip fracture group (n = 20) and blank control group (n = 4). Blood samples were taken at 4 h, 8 h, 24 h, 48 h and 72 h, respectively. The inflammatory factor IL-1 尾 -IL-6 + IL-10 TNF- 伪 mtDNA was detected quantitatively, and the animals were killed. Lung tissue, lung permeability coefficient, aquaporin concentration, dry wet weight ratio of lung tissue and HE staining were taken to study the relationship between mtDNA and inflammatory factors, and to provide a new theoretical basis for the treatment of elderly hip fracture complicated with lung injury. Result 1. The level of IL-1 尾 -IL-6 TNF- 伪 in peripheral blood of aged hip fracture rats increased gradually with the increase of time, reached the highest value at 24 hours, and then decreased gradually. At each time point, the level of IL-10 decreased to the lowest value at first to 24 hours, and then to the lowest value at each time point. The level of mtDNA in peripheral blood increased immediately after hip fracture trauma and reached a high value at 24 h, and then decreased. By correlation analysis, it was found that mtDNA was positively correlated with the level of inflammatory factor IL-6, and negatively correlated with the level of inflammatory factor IL-10. Compared with the control group, the lung tissue dry wet weight ratio and lung permeability index in the elderly hip fracture group were significantly higher than those in the control group. The results of western blotting showed that the AQP-1 of lung tissue was significantly lower than that of control group (P 0.05). After HE staining of lung tissue, some tissue cells were swollen, the cytoplasm was light stained, and the cell volume became larger. The alveolar septum was widened and there were more erythrocytes and neutrophils infiltrated in the alveolar cavity, and the lung pathological injury score was significantly higher than that in the control group. Conclusion (1) the content of IL-1 尾 IL-6 TNF- 伪 in peripheral blood of aged rats with hip fracture increased gradually with time, and reached its peak at 24 h. And then the content of IL-10 decreased gradually with the passage of time. After 24 hours, the content of IL-10 decreased to the lowest value in 24 hours, and then increased gradually. 2. The acute lung injury caused by hip fracture occurred in the aged rats, and the ratio of dry to wet weight of lung tissue increased. The expression of AQP-1 decreased with the increase of pulmonary permeability coefficient. The pathological manifestations were edema of lung tissue, intraalveolar hemorrhage, inflammatory cell infiltration, and the increase of peripheral blood mtDNA in elderly rats with hip fracture. And there was a positive correlation with the occurrence of inflammatory reaction.
