椎体成形术与椎体后凸成形术中骨水泥灌注压力的比较研究

【摘要】:目的:比较椎体成形术与椎体后凸成形术中骨水泥灌注压力的动态变化。方法:1.设计与调试能直接记录椎体成形术与椎体后凸成形术中骨水泥灌注压力动态变化的测量装置。2.在微机控制电子万能材料试验机上以0.25mm/min、40N,0.25mm/min、60N,0.5mm/min、60N各2组共6组进行验证试验,得到电压差与压力之间的关系。3.将10个椎体标本分成2组:PVP组(A1,A2,A3,A4,A5)、PKP组(B1,B2,B3,B4,B5),在室温20℃下经一侧椎弓根PVP组行椎体成形术而PKP组行椎体后凸成形术,骨水泥灌注量为2.4m L,灌注时间为48s,匀速地进行骨水泥灌注,采用设计的灌注压力测量装置记录骨水泥灌注时的实时动态压力。术后椎体行CT检查,观察骨水泥在椎体内的弥散分布情况。结果:1.验证试验中电压差与压力呈线性关系,压力随电压差的增加而变大,F(N)=440×U(V)。2.椎体后凸成形术中丙骨水泥的灌注压力较椎体成形术中降低。在室温20℃和推注速度为0.05m L/s下,PVP组第一管骨水泥的平均灌注压力为12.41±2.42N,最大灌注压力为19.89±3.82N,第二管骨水泥的平均灌注压力为19.81±3.03N,最大灌注压力为33.44±4.86N;而PKP组第一管骨水泥的平均灌注压力为6.33±2.05N,最大灌注压力为10.74±3.32N,第二管骨水泥的平均灌注压力为9.42±1.69N,最大灌注压力为18.18±1.77N。结论:1.椎体后凸成形术中骨水泥在椎体内灌注压力较椎体成形术中降低。2.椎体后凸成形术中骨水泥首先填充球囊扩张留下的空腔,灌注压力较低,弥散情况较椎体成形术中更为均匀,利于降低术中骨水泥渗漏的发生率。
[Abstract]:Objective: to compare the dynamic changes of bone cement perfusion pressure between vertebroplasty and kyphoplasty. Method 1: 1. Design and debug a measuring device that can directly record the dynamic changes of bone cement perfusion pressure in vertebroplasty and kyphoplasty.
