新型停搏液对在体心脏冠脉血管内皮保护效应的实验研究

【摘要】:目的:本文通过研究一氧化氮(NO)、内皮素(ET-1)和循环内皮细胞(CEC)水平变化,以及冠脉舒缩功能变化,探讨新型非去极化腺苷-利多卡因停搏液对在体犬冠脉血管内皮细胞的保护作用及其可能机制。方法:本研究分为两部分,第一部分建立犬体外循环模型并模拟临床体外循环下心脏直视手术,通过实验犬的生命体征和血流动力学等指标阐明犬体外循环手术的麻醉管理特点;第二部分12只实验杂种犬随机分为腺苷-利多卡因停搏液(Adenosine-lidocaine cardioplegia, AL组,n=6)和ST.thomasⅡ停搏液组(K组,n=6),测定术前(T1)、心脏复跳后1h(T2)和2h(T3)三个时点的一氧化氮(NO)、内皮素(ET)和循环内皮细胞(CEC)水平变化,并观察冠脉血管舒张功能变化,及进行透射电镜下超微结构分析。结果:两组术前基础值无统计学差异。T3时K组血浆中NO较AL组明显减少(P0.05),ET明显升高(P0.05),CEC明显增多(P0.05)。K组冠脉血管舒张最大百分比29.13%±8.24%低于AL组68.52%士11.32%(P0.01)。电镜结果提示:K组冠脉血管内皮细胞超微结构损伤较AL组更严重。结论:腺苷-利多卡因停搏液对犬冠脉血管内皮细胞保护效果优于ST.thomasⅡ停搏液。
[Abstract]:Objective: to study the changes of nitric oxide (no), endothelin (et 1) and circulating endothelial cell (CEC1) levels, and the changes of coronary systolic and diastolic function. To investigate the protective effect of a novel non-depolarized adenosine-lidocaine cardioplegic solution on canine coronary vascular endothelial cells (CAVEC) in vivo and its possible mechanism. Methods: this study was divided into two parts. The first part was to establish a canine cardiopulmonary bypass model and simulate the open heart surgery under clinical cardiopulmonary bypass. The characteristics of anesthetic management in dogs undergoing cardiopulmonary bypass (CPB) were elucidated by the vital signs and hemodynamics of experimental dogs.
