髋臼结构性植骨重建关节置换治疗髋臼发育不良伴骨关节炎

【摘要】：背景:髋臼发育不良是临床中常见髋关节疾病,可能引起患者髋关节的生物力学平衡受到破坏,加速髋关节机械磨损,可能诱发髋关节骨关节炎。人工全髋关节置换是现阶段治疗的首选手段。目的:探讨髋臼结构性植骨重建全髋关节置换对髋臼发育不良伴骨关节炎患者的临床应用效果。方法:50例髋臼发育不良伴骨性关节炎患者,依照Crowe标准,分为CroweⅠ型11例、CroweⅡ型16例、CroweⅢ型19例、CroweⅣ型4例,均采用髋臼结构性植骨重建全髋关节置换治疗;治疗前及治疗结束后6个月时观察并记录患者疼痛及功能恢复情况、Harris髋关节功能评分、生活质量评价。结果与结论:(1)治疗后CroweⅠ型、CroweⅡ型、CroweⅢ型、CroweⅣ型患者Harris评分均显著升高(P0.05);(2)治疗后所有患者疼痛及行走情况均显著改善(P0.05);(3)治疗后患者生理机能、生理职能、躯体疼痛、健康状况、精力、社会功能、情感职能及精神健康等生活质量有明显改善(P0.05);(4)结果说明,采用髋臼结构性植骨重建全髋关节置换髋臼发育不良伴骨关节炎,可使得髋臼杯放置更为合理,提供更好的髋臼假体支撑,恢复下肢长度并增加髋臼骨量,并对改善患者疼痛及髋关节功能,提高患者生活质量具有较好的临床疗效。
[Abstract]:Background: acetabular dysplasia is a common hip disease in clinic which may lead to the destruction of the biomechanical balance of the hip joint accelerate mechanical wear of the hip joint and may induce osteoarthritis of the hip joint.
