自制齿状钩钢板治疗后交叉韧带胫骨止点撕脱骨折

【摘要】：目的总结自制齿状钩钢板治疗后交叉韧带(posterior cruciate ligament,PCL)胫骨止点撕脱骨折的疗效。方法 2012年11月—2016年4月,采用自制齿状钩钢板治疗PCL胫骨止点撕脱骨折11例。其中男6例,女5例;年龄37~63岁,平均48.5岁。致伤原因:交通事故伤4例,高处坠落伤2例,跌伤5例;均为新鲜骨折,受伤至手术时间3~11 d,平均5.2 d。骨折按Meyer-Mc Keever分型:Ⅱ型4例,Ⅲ型7例。术前膝关节屈曲活动度40~90°,平均65.9°。术前Lysholm评分50~69分,平均58.5分。结果术后切口均Ⅰ期愈合。11例患者均获随访,随访时间6~20个月,平均11个月。术后3个月X线片示患者骨折均愈合,骨折端无明显移位。术后6个月,患肢膝关节屈曲活动度达115~130°,平均121.8°;膝关节后抽屉试验均呈阴性,未见膝关节伸直功能障碍及乆窝部血管、神经损伤并发症;Lysholm评分为85~99分,平均92.8分。结论自制齿状钩钢板治疗PCL胫骨止点撕脱骨折可行,操作简便、固定效果可靠,术后疗效满意。
[Abstract]:Objective to evaluate the efficacy of self-made dentate hook plate in the treatment of posterior cruciate ligament (cruciate) tibial avulsion fracture. Methods from November 2012 to April 2016, 11 cases of PCL tibial tibia avulsion fracture were treated with self-made dentate hook plate. There were 6 males and 5 females, aged 3763 years (mean 48.5 years). The causes of injury were as follows: 4 cases of traffic accident, 2 cases of falling injury and 5 cases of falling injury, all of them were fresh fractures, the average time from injury to operation was 5.2 days.
