自拟活血通脉汤联合外科手术对单纯性下肢浅静脉曲张患者氧化-抗氧化系统及血液流变学的影响

【摘要】：目的:观察自拟活血通脉汤联合外科手术对单纯性下肢浅静脉曲张患者氧化-抗氧化系统及血液流变学的影响。方法:将106例单纯性下肢浅静脉曲张患者随机分为观察组(n=53例)和对照组(n=53例),对照组给予外科手术治疗,观察组在对照组治疗基础上术后给予自拟活血通脉汤治疗,疗程为4周;观察两组治术后皮下血肿、硬结、瘀斑及患肢麻木疼痛等并发症的发生情况;观察两组治疗前后静脉疾病临床严重程度评分(RVCSS评分)、静脉功能不全生活质量问卷中的VEINES-Sym评分及VEINES-QOL评分、洪堡静脉曲张严重程度评分(HVVSS评分)变化情况;检测两组治疗前后氧化-抗氧化系统及血液流变学相关指标。结果:观察组治疗后2周、4周的皮下血肿、硬结、瘀斑及患肢麻木疼痛发生率低于对照组(均P0.05);两组治疗后2周、4周的RVCSS评分、VEINES-Sym评分、VEINES-QOL评分、HVVSS评分均有改善,观察组上述评分的改善情况优于对照组(均P0.05);观察组治疗后2周、4周的临床疗效优于对照组(均P0.05);两组治疗后过氧化氢酶(CAT)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-PX)升高,丙二醛(MDA)水平降低,观察组上述指标改善情况优于对照组(均P0.05);观察组治疗后全血黏度(低、中、高切)、血浆黏度及红细胞比积均有低于对照组(均P0.05)。结论:外科手术治疗单纯性下肢浅静脉曲张的基础上,采用自拟活血通脉汤治疗能够降低术后并发症,并改善患者症状,提高疗效及生活质量,抑制氧化应激反应并改善血液流变学,从而促进患者康复。
[Abstract]:Objective: to observe the effect of Huoxue Tongmai decoction combined with surgery on oxidation-antioxidation system and hemorheology in patients with simple superficial varices of lower extremity. Methods: 106 patients with simple superficial varicose veins of lower extremity were randomly divided into two groups: the observation group (n = 53) and the control group (n = 53). The control group received surgical treatment. The observation group was treated with self-made Huoxue Tongmai decoction after operation on the basis of the treatment in the control group. The course of treatment was 4 weeks. The complications such as subcutaneous hematoma, induration, ecchymosis and numbness were observed in the two groups. Before and after treatment, the clinical severity scores of venous diseases, VEINES-Sym scores and VEINES-QOL scores, and Humboldt variceal severity scores were observed. The oxidation-antioxidation system and hemorheology were measured before and after treatment in both groups. Results: the incidence of subcutaneous hematoma, induration, ecchymosis and limb numbness in the observation group was lower than that in the control group 2 weeks after treatment (all P 0.05), the RVCSS score and VEINES-system score and VEINES-QOL score were improved in both groups 2 weeks after treatment and 4 weeks after treatment. The improvement of the above scores in the observation group was better than that in the control group (all P 0.05), the clinical effect in the observation group was better than that in the control group at 2 weeks and 4 weeks after treatment (all P 0.05), and the levels of catalase, superoxide dismutase (SOD) and glutathione peroxidase (GSH-PX) in the two groups were increased after treatment. The level of malondialdehyde (MDA) decreased, the improvement of above indexes in the observation group was better than that in the control group (all P 0.05), the whole blood viscosity (low, medium, high shear rate), plasma viscosity and hematocrit in the observation group were lower than those in the control group (all P 0.05). Conclusion: on the basis of surgical treatment of simple superficial varices of lower extremity, self-made Huoxue Tongmai decoction can reduce postoperative complications, improve symptoms, improve curative effect and improve quality of life. Inhibit oxidative stress and improve hemorheology, thus promoting patient recovery.
