骨质疏松性胸腰椎体压缩性骨折患者外科治疗后腰背部位疼痛研究

【摘要】：目的探讨骨质疏松性胸腰椎体压缩性骨折患者外科治疗后腰背部位疼痛的相关因素。方法回顾性分析2013年1月至2016年1月该院收治的621例骨质疏松性胸腰椎体压缩性骨折患者的临床资料,所有患者均采用经皮球囊扩张后凸成形术(PKP)或经皮椎体成形术(PVP)治疗。Logistic回归分析骨质疏松性胸腰椎体压缩性骨折患者外科治疗后腰背部位疼痛的危险因素。结果治疗后患者的视觉模拟评分法(VAS)评分和Oswestry功能障碍指数评分较治疗前均显著降低,差异有统计学意义(P0.05)。621例患者中94(15.13%)例患者治疗后腰背部位疼痛。术后发生骨水泥渗漏及骨水泥渗漏的方向、体积、范围和并发症是骨质疏松性胸腰椎体压缩性骨折患者外科治疗后腰背部位疼痛的危险因素(P0.05)。结论 PKP和PVP治疗能够显著缓解患者的疼痛,但术后发生骨水泥渗漏和并发症会造成患者术后腰背部位疼痛,应采取积极措施降低术后腰背部位疼痛的发生率。
[Abstract]:Objective to investigate the related factors of lumbar dorsal pain after surgical treatment in patients with osteoporotic thoracolumbar vertebral compression fracture. Methods the clinical data of 621 patients with osteoporotic thoracolumbar vertebral compression fracture treated in our hospital from January 2013 to January 2016 were retrospectively analyzed. All patients were treated with percutaneous balloon kyphoplasty (PKP) or percutaneous vertebroplasty (PVP). Logistic regression analysis was performed on the risk factors of pain in the posterior lumbar area of patients with osteoporotic thoracolumbar vertebral compression fracture after surgical treatment. Results after treatment, the visual analogue score (VAS) and the Oswestry dysfunction index (Oswestry) score were significantly lower than those before treatment, and the difference was statistically significant (P 0.05. 621). The direction, volume, scope and complications of bone cement leakage and bone cement leakage were the risk factors of pain in the lumbar back after surgical treatment in patients with osteoporotic thoracolumbar vertebral compression fracture. Conclusion PKP and PVP can significantly relieve the pain of the patients, but the occurrence of bone cement leakage and complications will result in postoperative pain in the back of the waist, so we should take active measures to reduce the incidence of postoperative pain in the back of the back.
