具有抗感染功能的硬膜外—皮下埋藏式镇痛泵的研制

【摘要】:目的: 本课题旨在研制一种具有抗感染功能的硬膜外-皮下埋藏式镇痛泵,评价其抗感染性能。 方法: 设计具有抗感染功能的硬膜外-皮下埋藏式镇痛泵的图纸。其主要部件皮下埋藏式注药壶,采用堇青石多微孔陶瓷内胆,微孔孔径通过粘土粉体与医用石蜡以质量比调节。 采用化学还原的方法,以硼氢化钠为还原剂,壳聚糖为稳定剂,在硝酸银溶液内制备壳聚糖/纳米银复合溶液。堇青石陶瓷内胆表面定量负载原位法生成的壳聚糖/纳米银抗菌涂层。 对壳聚糖/纳米银抗菌涂层的堇青石进行抗大肠杆菌和金黄色葡萄球菌的体外抑菌试验,分别用原位生成法和外加纳米银液制备壳聚糖/纳米银抗菌涂层,定量负载到滤纸片和堇青石样品上,观察体外抑菌效果;同时行抑菌持续时间检测,将壳聚糖/纳米银抗菌涂层的滤纸片和堇青石样品分别浸泡于磷酸缓冲盐溶液,三蒸水和10%吗啡溶液中30天后取出,测试其耐水洗性和抗菌性能(抑菌带)。每次试验均重复3次。 结果: 成功设计了具有抗感染功能的硬膜外-皮下埋藏式镇痛泵的图纸。其主要部件皮下埋藏式注药壶,采用堇青石多微孔陶瓷内胆,微孔孔径可调节,当粘土粉体与医用石蜡为100:13时,所得陶瓷内胆的孔隙率约为26.4%,最可几孔径尺寸为Φ6.6μm。 体外抑菌试验显示,本研究使用的原位生成法相比于外加纳米银法能够有效增强纳米银抗菌效果,使得壳聚糖/纳米银抗菌涂层抗菌能力更强;壳聚糖/纳米银涂层的堇青石具有良好的抗大肠杆菌和金黄色葡萄球菌效果;且有一定的耐水洗效果,抑菌持续时间至少可达30天。 结论: 本课题成功研制了一种具有良好抗感染功能的硬膜外-皮下埋藏式镇痛泵。体外实验结果表明壳聚糖/纳米银涂层的堇青石多微孔陶瓷内胆抑菌持续时间至少可达30天,能有效预防硬膜外导管长期留置引起的相关感染。
[Abstract]:Objective: The purpose of this paper is to develop an anti-infective epidural-subcutaneous analgesic pump and evaluate its anti-infection performance. Methods: Design the drawing of epidural-subcutaneous analgesic pump with anti-infection function. The main component of the pot is subcutaneously implanted. Cordierite multiporous ceramic inner bile is used. The pore diameter of cordierite is adjusted by the mass ratio of clay powder to medical paraffin. Chitosan / nano-silver composite solution was prepared in silver nitrate solution by chemical reduction with sodium borohydride as reducing agent and chitosan as stabilizer. Chitosan / nano silver antibacterial coating prepared by quantitative loading on cordierite ceramic inner bile surface. In vitro bacteriostatic tests were carried out on cordierite coated with chitosan / nano-silver antibacterial coating against Escherichia coli and Staphylococcus aureus. Chitosan / nano-silver antibacterial coating was prepared by in-situ formation and addition of nano-silver solution, respectively. The bacteriostatic effect was observed by loading the filter paper and cordierite sample quantitatively, and the bacteriostatic duration was determined. The filter paper and cordierite sample of chitosan / nano-silver antibacterial coating were immersed in phosphate buffer solution, respectively. The three distilled water and 10% morphine solution were taken out after 30 days to test their washability and antimicrobial activity. Each trial was repeated 3 times. Results: The drawing of epidural-subcutaneous analgesic pump with anti-infection function was successfully designed. The main component of the pot is subcutaneously implanted, and cordierite multi-porous ceramic inner bile is used. The pore size of the ceramic is adjustable. When the ratio of clay powder and medical paraffin is 100: 13, the porosity of ceramic inner bile is about 26.4 and the most likely pore size is 桅 6.6 渭 m. The bacteriostatic test in vitro showed that the in-situ formation method used in this study could effectively enhance the antibacterial effect of nano-silver and make the antibacterial ability of chitosan / nano-silver antibacterial coating stronger than the addition of nano-silver method. The cordierite coated with chitosan / nano-silver has good resistance to Escherichia coli and Staphylococcus aureus, and has a certain anti-washing effect, and the bacteriostatic duration can reach at least 30 days. Conclusion: A kind of epidural-subcutaneous analgesic pump with good anti-infection function has been successfully developed. The results showed that the bacteriostatic time of cordierite microporous ceramic coated with chitosan / nano-silver coating was at least 30 days, which could effectively prevent the infection caused by long-term indwelling of epidural catheter.
