人体第6、7、8肋肋软骨三维测量及骨化研究

【摘要】:[目的]分析不同年龄不同性别人群第6、7、8肋肋软骨三维(长、宽、厚)情况及骨化程度,为临床隆鼻及耳廓再造等选用肋软骨术前评估及术后远期预后估计提供参考。[方法]回顾分析昆明医科大学第二附属医院放射科2014年5月到2016年3月320例行胸部CT检查的6岁到45岁患者,5岁为1个年龄组,分成8组:6-10岁,11-15岁,16-20岁,21-25岁,26-30岁,31-35岁,36-40岁,41-45岁。每组40例患者,男性20例,女性20例。运用多层螺旋CT (MSCT)检查及飞利浦工作站提供的多种图像后处理技术:容积再现(volume rendering technique,VRT)、最大密度投影法(maximum intensity projection, MIP)、最小密度投影法(minimum intensity projection, Min IP)、表面阴影显示法(shaded surface display, SSD)、多平面重组(multiple planar re-construction, MPR)、曲面重组(Curved planar reformat, CPR)等,对第6、7、8肋肋软骨进行CT值测量,得出三维数值及骨化体积、肋软骨体积等相关数据,分析肋软骨骨化程度。采用SPSS17.0软件进行统计学分析,观察指标以均数±标准差(xs)表示。以P0.05为差异具有统计学意义。[结果]数据统计结果提示,第6肋软骨右侧长度为(101.359 0±13.8962)mm,宽度为(12.119±3.3645)mm,厚度为(8.647±1.6501)mm。第6肋软骨左侧长度为(106.200±17.1694)mm,宽度为(12.373±3.312)mm,厚度为(8.744±1.4668)mm。第7肋软骨右侧长度为(140.469±18.9291)mm,宽度为(11.55±2.878)mm,厚度为(7.636±2.4719)mm。第7肋软骨左侧长度为(143.309±20.3027)mm,宽度为(11.666±3.1590)mm,厚度为(7.677±1.9541)mm。第8肋软骨右侧长度为(106.39±39.105)mm,宽度为(8.448±2.7122)mm,厚度为(6.553±1.6221)mm。第8肋软骨左侧长度为(107.86±42.212)mm,宽度为(8.422±2.7895)mm,厚度为(5.272±1.9027)mm。[结论]1.人体第6、7、8肋肋软骨的三维测量数据变化以20岁为界。20岁以前,肋软骨的三维值随着年龄增长而增大,20岁以后基本不变。在所观察的肋软骨中,第7肋肋软骨具有最大的厚度,长度和宽度,三维数据稳定,骨化程度适中,可作为整形外科行鼻整形或耳再造首选肋软骨。2.人体第6、7、8肋肋软骨骨化从15岁开始,骨化率和骨化程度随年龄增长而增加。第6、7、8肋肋软骨程度从上到下依次递减。3.从肋软骨的体积考虑,建议10岁以后再行肋软骨切取的整形手术。从骨化角度来看,到45岁,依旧可行肋软骨切取的整形手术,因年龄增长而增加的骨化不是行肋软骨切取整形手术的绝对禁忌症。
[Abstract]:[objective] to analyze the three dimensional (length, width, thickness) and ossification degree of costal cartilage in the 6th and 8th costal cartilage of different ages and different genders, so as to provide reference for clinical evaluation of costal cartilage before operation and long term prognosis estimation for auricle reconstruction. [methods] from May 2014 to March 2016, 320 patients aged from 6 to 45 years old who received chest CT examination in Department of Radiology, second affiliated Hospital of Kunming Medical University were analyzed retrospectively. They were divided into 8 groups: 6-10 years old, 11-15 years old, 16-20 years old, 16-25 years old, 26-30 years old, 31-35 years old and 36-40 years old and 41-45 years old. There were 40 patients in each group, 20 males and 20 females. Using multi-slice spiral CT scan and various image post-processing techniques provided by Philips workstation: volume rendering technique, maximum intensity projection, minimum intensity projection, Min projection, shaded surface display. SSD, Multiplanar recombination multiple planar re-construction, MPRA, curved Surface recombination, Curved planar reformat, CPR), etc. The CT value of the 6th and 8th costal cartilage was measured, and the three-dimensional value, ossification volume and costal cartilage volume were obtained, and the ossification degree of the costal cartilage was analyzed. The statistical analysis was carried out by SPSS17.0 software, and the observed index was expressed as mean 卤standard deviation xs. P0.05 as the difference was statistically significant. [results] the data showed that the length of the right side of the sixth costal cartilage was 101.359 0 卤13.8962mm, the width was 12.119 卤3.3645mm, and the thickness was 8.647 卤1.6501mm. The left length, width and thickness of the sixth costal cartilage were 106.200 卤17.1694 mm, 12.373 卤3.312 mm and 8.744 卤1.4668 mm. respectively. The length of the right side of the seventh costal cartilage was 140.469 卤18.9291mm. the width was 11.55 卤2.878mm. the thickness was 7.636 卤2.4719mm. The left length of the seventh costal cartilage was 143.309 卤20.3027 mm, the width was 11.666 卤3.1590 mm, and the thickness was 7.677 卤1.9541 mm. The length, width and thickness of the eighth costal cartilage were 106.39 卤39.105 mm, 8.448 卤2.7122 mm and 6.553 卤1.6221 mm respectively. The left length of the eighth costal cartilage was 107.86 卤42.212mm, the width was 8.422 卤2.7895mm, the thickness was 5.272 卤1.9027mmm. [conclusion] 1. The change of 3D measurement data of costal cartilage in the 6th and 8th chondrocytes of human body. Before the age of 20 years, the three-dimensional value of the costal cartilage increases with the increase of age, and after 20 years of age, the value of the costal cartilage is almost unchanged. Among the costal cartilage observed, the seventh costal cartilage has the largest thickness, length and width, the three-dimensional data is stable, and the ossification degree is moderate. It can be used as the first choice of costal cartilage for orthopedic surgery or ear reconstruction. The ossification rate and the degree of ossification of costal cartilage began at the age of 15 years, and the rate of ossification and the degree of ossification increased with age. The degree of costal cartilage decreased from top to bottom. In view of the volume of costal cartilage, it is recommended that plastic surgery should be performed after 10 years of age. From the point of view of ossification, the plastic surgery of costal cartilage removal is still feasible at the age of 45, and the increase of ossification due to age is not an absolute contraindication for the operation of costal cartilage removal.
