非哺乳期乳腺炎发病的相关因素分析

【摘要】：乳腺炎是由于多种因素的影响导致以乳腺脓肿为主要症状的妇科常见炎症，多发于产后哺乳期，部分发生于青春期、老年期等非哺乳阶段。乳腺炎根据其发病的时期、临床症状的不同分为哺乳期（LA）和非哺乳期乳腺炎（NLA）。临床上LA的发病原因较为确定，多为乳汁淤积、细菌感染等引起的乳腺小叶的炎症，其治愈率较高。而NLA的发病因素较为复杂，其临床症状通常表现为瘘管、乳头内陷、乳头溢液等，根据其病理学的不同表现又将NLA分为乳腺周围炎（PDM）、导管扩张症（MDE）和小叶肉芽肿性乳腺炎（GLM）等类型，由于病因不明、病情复杂，因此NLA在临床上具有难治愈、易复发的特点，严重影响患者的心理、生理健康和生活质量。学界上对于NLA致病因素的研究也未有明确的结论，本研究对PDM、MDE和GLM三种不同的NLA的影响因素进行Logistic相关分析，并针对影响因素提出具体的预防措施和相应的注意事项，对于NLA的预防和治疗具有重大临床意义。研究的主要内容为对本院2012年1月～2014年1月期间收入治疗的NLA患者的一般资料进行回顾性分析以及统计，通过文献调研选择相关可能影响因素，分别以患者年龄、哺乳经验、分娩经历、乳腺肿物、导管扩张、化学刺激、免疫疾病、乳腺创伤、遗传疾病、细菌感染、嗜烟习惯以及泌乳素水平增高等因素作为自变量，以上因素在我院PDM、MDE和GLM患者中的对应发生率作为因变量建立Logistic模型分析三种NLA发病的相关影响因素；并根据分析结果总结预防措施。以发病类型、治疗方案、护理满意度情况和患者心理状况等影响因子为自变量，，总有效率为因变量建立Logistic模型分析影响疗效的因素，探讨临床治疗中所需注意的事项。结果显示，哺乳经验、分娩经历、乳腺肿物、乳头内陷、细菌感染、吸烟习惯为PDM影响因素；导管扩张、乳腺肿物、年龄情况、细菌感染、乳管组织化学性刺激为MDE发病相关因素；GLM发病因素相对复杂，哺乳经验、分娩经历、乳腺肿物、细菌感染、免疫疾病、创伤、遗传病史、泌乳素增高均为相关性影响因素。纠正不良的生活习惯、合理选用药物、积极调节内分泌状况可有效预防各类NLA的发生。最后通过对NLA疗效的相关影响因素进行分析发现，在治疗前对患者的生理指标情况、NLA发病类型、症状情况、病史资料等进行合理的评估，并提升心理干预、基础护理的质量和选择正确的治疗方案对于提升NLA的治疗效果具有关键作用。
[Abstract]:Mastitis is a kind of common inflammation caused by various factors , such as mammary abscess as the main symptom , which is mostly in the lactation period of adolescence , old age and other non - feeding stages . The reason of clinical symptoms is complicated . The clinical symptoms usually show that fistula , nipple collapse , nipple discharge and so on . The clinical symptoms usually appear as fistula , nipple collapse , nipple discharge , etc .

The main contents of this study were to analyze the general data of NLA patients who received income treatment from Jan . 2012 to January 2014 , and select the relevant factors by literature investigation . The factors influencing the incidence of NLA were analyzed by Logistic model based on the corresponding incidence rate of the patient ' s age , breast feeding experience , labor experience , breast mass , catheter dilatation , chemical stimulation , immune disease , breast trauma , genetic disease , bacterial infection , tobacco habit and prolactin level .
Based on the analysis results , the preventive measures were summarized . The factors influencing the curative effect of the Logistic model were established based on the type of incidence , the treatment plan , the satisfaction of nursing and the psychological condition of the patient , and the factors which need to be paid attention in the clinical treatment were discussed .

The results showed that nursing experience , labor experience , breast swelling , nipple sinkage , bacterial infection , smoking habit were the influencing factors of PDM ;
Catheter dilatation , breast mass , age , bacterial infection , chemical stimulation of breast tube tissue were related factors of the disease .
GLM pathogenesis is relatively complex , nursing experience , labor experience , breast swelling , bacterial infection , immune disease , trauma , genetic disease history and prolactin increase are related factors . Correct bad habits of living , select drugs rationally , actively regulate endocrine condition can effectively prevent various NLA ' s occurrence .

Finally , through the analysis of the factors related to the efficacy of NLA , the physiological indexes of the patients , the type of NLA , the symptoms and the history data of the patients were assessed rationally before the treatment , and the psychological intervention , the quality of the basic nursing and the choice of the correct treatment scheme have a key role in improving the treatment effect of NLA .
