腹膜前间隙与疝环填充式无张力修补治疗股疝的临床疗效对比分析

【摘要】：目的：探讨经腹膜前间隙无张力腹股沟疝修补术与疝环填充式无张力腹股沟疝修补术治疗腹股沟股疝的临床疗效。方法：回顾性分析2009年1月—2014年12月在新疆医科大学第一附属医院收治的腹股沟股疝患者23例,根据手术方式,分为经腹膜前间隙无张力腹股沟股疝修补术与疝环填充式无张力腹股沟股疝修补术进行临床对比研究。观察两组平均手术时间、术后疼痛VAS评分、术后并发症及术后复发情况。结果：腹膜前间隙组与疝环填充式组手术平均时间分别为44.1±5.68min和39.6±4.86min;术后疼痛4、8、12h二组疼痛VAS差异无统计学意义(P0.05),但术后1个月疼痛VAS差异有统计学意义(P0.05)。腹膜前间隙组阴囊水肿1例,术后4天消退,无术后异物感,无术后复发；疝环填充式组术后阴囊水肿2例,术后3-7天消退,皮下血清肿2例,经穿刺后消失。术后异物感5例,随访5-18个月无术后复发情况,二者术后异物感有统计学意义(P0.05)。结论：经腹膜前间隙无张力修补术治疗腹股沟股疝就术后异物感而言优于疝环填充式修补,且经腹膜前间隙无张力修补术应用补片覆盖整个肌耻骨孔,更能预防其它疝的复发,是治疗腹股沟股疝更为合理的手术方式。
[Abstract]:Objective: to investigate the clinical effect of tension-free inguinal hernioplasty via anterior peritoneal space and tension-free hernia repair with hernia ring filling in the treatment of inguinal femoral hernia. Methods: from January 2009 to December 2014, 23 patients with inguinal hernia were treated in the first affiliated Hospital of Xinjiang Medical University. A comparative study was carried out between tension-free inguinal hernia repair and tension-free inguinal hernia repair. The mean operation time, postoperative pain VAS score, postoperative complications and postoperative recurrence were observed. Results: the mean operation time of anterior peritoneal space group and hernia ring filling group was 44.1 卤5.68min and 39.6 卤4.86 minutes, respectively, but there was no significant difference in pain between the two groups at 812 hours after operation (P 0.05), but there was significant difference in pain 1 month after operation (P 0.05). In the preperitoneal space group, scrotum edema disappeared 4 days after operation, no postoperative foreign body sensation, no recurrence, and scrotal edema disappeared in 2 cases, 3 to 7 days after operation in hernia ring filling group, and subcutaneous serum swelling disappeared after puncture. 5 cases of postoperative foreign body sensation were followed up for 5 to 18 months, there was no recurrence after operation, there was significant difference between them (P 0.05). Conclusion: the treatment of inguinal femoral hernia via anterior peritoneal space tension-free repair is superior to that of hernia ring filling repair in terms of postoperative foreign body sensation, and the application of patch covering the whole muscle pubic foramen can prevent the recurrence of other hernias. It is a more reasonable way to treat inguinal femoral hernia.
