乙酰肝素酶mRNA在乳腺癌中的表达及其与乳腺癌病理特征的关系

【摘要】:背景 乙酰肝素酶(HPA)是一种葡萄糖醛酸脂酶,其能够在特定部位裂解硫酸乙酰肝素蛋白聚糖(heparin sulfate proteoglycan, HPS G)中的硫酸乙酰肝素(HS)链,进而导致细胞外基质(extra cellular matrix, ECM)和基底膜(basement membrane,BM)发生降解,促进肿瘤的浸润、转移与微血管形成。乳腺癌是我国比较多发的恶性肿瘤之一,目前国内有关于HPA mRNA是否在乳腺癌中表达的情况未见报道,也无文献报道其是否与肿瘤的侵袭性生长、淋巴结转移和血管形成有关。本研究采用原位杂交方法和肿瘤微血管免疫组织化学技术检测50例乳腺癌组织中的HPA mRNA的表达和肿瘤微血管密度microvessel density, MVD),对 HPA mRNA在乳腺癌组织中的表达及其临床意义进行探讨。目的探讨乙酰肝素酶(HPA) mRNA及CD34在乳腺癌组织中的表达,分析HPAmRNA与乳腺癌病理特征及MVD的关系。方法收集新乡医学院第一附属医院2013年2~9月手术切除的50例乳腺癌组织标本及50例相应癌旁正常乳腺组织(距离肿瘤边缘5cm以上)标本,患者均为女性,年龄23~76岁,中位年龄56岁,患者术前均未接受放射治疗、化学治疗及内分泌治疗;术后常规病理切片均证实为乳腺癌。按国际抗癌协会分类标准:Ⅰ级14例,Ⅱ级26例,Ⅲ级10例;Ⅰ期10例,Ⅱ24例,Ⅲ期16例;N018例,N120例,N29例,N33例。每例标本均分为2份,一份用液氮保存,待提取组织RNA;另一份应用40 g·L-1多聚甲醛固定,生物自动脱水机处理后石蜡包埋,行免疫组织化学染色。采用RT-PCR检测HPA mRNA的表达情况及免疫组织化学方法检测CD34的表达情况。应用SPSS19.0软件进行统计分析,计量资料以均数±标准差(x±s)表示,两两比较采用t检验,计数资料比较采用χ2检验,检验水准为α=0.05。结果通过统计分析对比,乳腺癌组织和癌旁正常乳腺组织中HPA mRNA阳性表达率分别为70.0%(35/50)和24.0%(12/50),乳腺癌组织中HPA mRNA阳性表达率显著高于癌旁正常乳腺组织(P0.01); HPA mRNA的表达与乳腺癌组织学分级、淋巴结转移、TNM分期及肿瘤侵袭程度有相关性(P0.05,P0.01),与患者的年龄无关(P0.05);CD34阳性表达主要位于血管内皮细胞的胞膜和胞质,呈棕黄色;35例HPA mRNA阳性患者为MVD为58.20±18.56,15例HPA mRNA阴性患者MVD为41.30±9.35, HPA mRNA阳性患者MVD显著高于HPA mRNA阴性患者(P0.01)。结论HPA mRN A在乳腺癌组织中的表达显著增高;HPA mRNA表达阳性的乳腺癌组织内微血管密度高于其阴性表达者,其能够促进乳腺癌新生血管形成HPA mRNA高表达参与乳腺癌发展过程,与乳腺癌进展、转移密切相关,为临床上的肿瘤筛选及预后提供生物学标记作为依据。
[Abstract]:Background Heparinase (HPA) is a glucuronidase that cleavage heparin sulfate proteoglycan (HPSG) chain of heparin sulfate in a specific site. In turn, extracellular matrix extra cellular matrix (ECM) and basement membrane BM2 were degraded, which promoted tumor invasion, metastasis and microangiogenesis. Breast cancer is one of the most frequent malignant tumors in China. At present, there are no reports on whether HPA mRNA is expressed in breast cancer, or whether it is related to invasive growth, lymph node metastasis and angiogenesis. In this study, the expression of HPA mRNA and tumor microvessel density (MVD) in 50 cases of breast cancer were detected by in situ hybridization and immunohistochemical technique. The expression of HPA mRNA in breast cancer and its clinical significance were studied. Objective to investigate the expression of heparanase HPA mRNA and CD34 in breast cancer, and to analyze the relationship between HPA mRNA and pathological features and MVD in breast cancer. Methods 50 breast cancer tissues and 50 adjacent normal breast tissues (above the margin of 5cm) were collected from the first affiliated Hospital of Xinxiang Medical College from February to September 2013. The patients were all female, aged 2376 years. The median age was 56 years old. The patients were not treated with radiotherapy, chemotherapy and endocrine therapy before operation.
