急性闭合性跟腱断裂微创治疗的中、远期疗效观察

【摘要】：目的:通过中、远期随访观察,评价微创经皮缝合手术治疗急性闭合性跟腱断裂的临床疗效与价值。方法:随访我院自2012年9月—2017年1月33例接受微创手术治疗的急性闭合性跟腱断裂患者,平均随访2.02年(3个月—5年),以门诊随访结合电话随访,通过观察切口情况、活动功能情况、有无再断裂发生,并应用AOFAS评分标准对术后疗效进行评价。结果:术后4例失访,随访时间3个月~5年,平均2.02年。3所有病例无深静脉血栓发生,无腓肠神经支配区感觉异常,无切口愈合不良、感染、皮缘坏死等切口并发症发生;随访期内无跟腱再断裂发生病例。术后中远期足踝外形、功能恢复好,疗效评定为:随访时间3个月至1年共11例,AOFAS评分平均92.63分,其中优4例,良4例,优良率72.7%;随访时间1年以上共22例,AOFAS评分平均96.33分,其中优19例,良2例,优良率95.5%。结论:微创经皮缝合手术是治疗急性闭合性跟腱断裂较好的方法,术后中远期并发症少、外形美观、功能恢复良好。
[Abstract]:Objective: to evaluate the clinical effect and value of minimally invasive percutaneous suture for acute closed rupture of Achilles tendon. Methods: from September 2012 to January 2017, 33 patients with acute closed Achilles tendon rupture received minimally invasive surgery were followed up in our hospital from September 2012 to January 2017. The patients were followed up for an average of 2. 02 years (3 months to 5 years). The activity function and the occurrence of rerupture were evaluated by AOFAS score. Results: the follow-up time was 3 months to 5 years. No deep venous thrombosis, no sensory abnormality in the sural nerve innervation area, no wound healing, infection, skin edge necrosis and other incision complications occurred in 4 cases. No recurrent rupture of Achilles tendon occurred during the follow-up period. The average score of AOFAS was 92.63 points in 11 cases from 3 months to 1 year, of which 4 cases were excellent, 4 cases were good, the excellent and good rate was 72.7%, and the average AOFAS score was 96.33 points in 22 cases during the follow-up period of more than one year. Among them, 19 cases were excellent, 2 cases were good, the excellent and good rate was 95.5%. Conclusion: minimally invasive percutaneous suture is a better method for the treatment of acute closed rupture of Achilles tendon.
