后踝骨折的损伤机制及手术治疗结果分析

【摘要】：[目的]探讨后踝骨折的损伤机制,并对33例患者临床治疗效果进行分析。[方法]回顾性分析本院2012年1月~2014年2月收治的33例单侧后踝骨折患者病例,分析患者术前X线片、CT结果,进行Danis-Weber分型、Lauge-Hansen分型、Haraguchi分型,并对后踝骨折的损伤机制进行判断。术后采用Baird-Jackson踝关节功能评定标准对手术疗效进行评价。[结果]根据患者影像资料,将骨折分型:Danis-Weber:A型0例、B型21例、C型12例;Haraguchi:Ⅰ型14例、Ⅱ型19例、Ⅲ型0例;Lauge-Hansen:旋后外旋19例、旋前外旋3例、旋后内收1例、无法分型10例。通过分析,作者认为后踝骨折的损伤机制可能有垂直应力、旋转应力、内收应力、混合应力等。术后随访9~14个月,骨折均获骨性愈合。末次随访Baird-Jackson评分:优21例、良8例、中4例。[结论]后踝骨折损伤机制复杂多样,临床治疗应依据损伤情况合理选择手术入路进行固定治疗。
[Abstract]:Objective: to investigate the mechanism of posterior ankle fracture and to analyze the clinical effect of 33 cases. [methods] 33 cases of unilateral posterior malleolar fracture from January 2012 to February 2014 were retrospectively analyzed. The results of X-ray and CT were analyzed before operation, and Haraguchi classification was performed for Danis-Weber classification and Lauge-Hansen classification, and the injury mechanism of posterior malleolus fracture was judged. Postoperative Baird-Jackson ankle function evaluation criteria were used to evaluate the outcome of the operation.
