关节镜下outside-in技术修复半月板前角损伤的临床疗效观察

【摘要】：目的:探讨关节镜下应用outside-in技术修复半月板前角损伤的缝合方法及其疗效。材料与方法:对沈阳市骨科医院2014年9月至2016年5月间的36例半月板前角损伤的患者,行关节镜下outside-in技术修复半月板前角撕裂。对于合并前交叉韧带断裂的患者取自体乆绳肌腱重建前交叉韧带。术前及术后应用Lysholm评分、国际膝关节文献委员会膝关节评估表(IKDC)以及疼痛视觉模拟评分(VAS)对手术疗效进行评价。通过Barrett标准、单腿跳远测试进行临床客观评定。结果:1.所有患者随访9~29个月,平均19.1个月;2.根据Barrett标准评价半月板临床愈合情况,33例临床愈合,临床愈合率为91.67%(33/36);3.术后末次随访患肢单腿跳远测试距离为(136.67±17.45)cm,33例患者达到正常肢体的对称性;4.术后Lysholm评分、IKDC评分及VAS评分均明显高于术前,Lysholm评分:术前(63.72±9.81)分,术后(86.36±7.91)分;IKDC评分:术前(45.08±6.78)分,术后(89.61±4.83)分;VAS评分:术前(5.19±0.98)分,术后(0.75±0.87)分,差异均有统计学意义(P0.05);5.术后Lysholm评分总优良率为91.7%。结论:关节镜下outside-in技术修复半月板前角损伤,方法简单,不需要特殊器械,并发症少,安全可靠,临床愈合率高,术后患者满意率高,疗效显著。
[Abstract]:Objective: to study the suture method and curative effect of arthroscopic outside-in technique for meniscus anterior horn injury. Materials and methods: from September 2014 to May 2016, 36 patients with meniscus anterior angle injury were treated with arthroscopic outside-in technique to repair anterior meniscus tear. Anterior cruciate ligament reconstruction was performed in patients with anterior cruciate ligament rupture. Before and after operation, Lysholm score, IKDCand pain visual analogue scale (VAS) were used to evaluate the curative effect of the operation. Objective clinical evaluation was carried out by Barrett standard and single leg long jump test. The result is 1: 1. All patients were followed up for 9 to 29 months, with an average of 19.1 months. According to Barrett criteria, 33 cases of meniscus healing were evaluated. The clinical healing rate was 91.67%. After the last follow-up, the test distance of single leg long jump was 136.67 卤17.45 cm ~ (-1) in 33 patients with normal limb symmetry. The postoperative Lysholm score and VAS score were significantly higher than those of preoperative Lysholm score (63.72 卤9.81), postoperatively (86.36 卤7.91) and IKDC (45.08 卤6.78), VAS (89.61 卤4.83), pre-operation (5.19 卤0.98) and postoperatively (0.75 卤0.87), respectively. The total excellent and good rate of Lysholm score after operation was 91.7%. Conclusion: arthroscopic outside-in technique for meniscus anterior horn injury repair is simple, without special instruments, less complications, safe and reliable, high clinical healing rate, high satisfaction rate and significant curative effect.
