前交通动脉动脉瘤栓塞术后症状性血管痉挛影响因素的临床分析

【摘要】：目的：探讨前交通动脉动脉瘤患者血管内栓塞术后症状性脑血管痉挛的影响因素。 方法：分析吉林大学白求恩第一医院收治的70例前交通动脉瘤患者的临床资料,用卡方检验及logistic回归分析法分析症状性脑血管痉挛的影响因素。 结果：经过卡方分析,症状性脑血管痉挛的发生率与蛛网膜下腔出血的Fisher分级，入院时病情Hunt-Hess分级，前交通动脉动脉瘤干预的时间有关。与年龄，性别，高血压病史，动脉瘤的大小，支架辅助栓塞。经过logistic回归分析法分析，Fisher分级及手术时机是症状性脑血管痉挛的独立的影响因素。 结论：根据其影响因素，选择恰当的干预时机及合适的干预方式，获得理想的治疗效果。支架辅助栓塞是否是脑血管痉挛的影响因素，，有待进一步研究。
[Abstract]:Objective: to investigate the influencing factors of symptomatic cerebral vasospasm in patients with anterior communicating aneurysm after endovascular embolization. Methods: the clinical data of 70 patients with anterior communicating artery aneurysm treated in Bai Qiuen first Hospital of Jilin University were analyzed. The influencing factors of symptomatic cerebral vasospasm were analyzed by chi-square test and logistic regression analysis. Results: through chi-square analysis, the incidence of symptomatic cerebral vasospasm was related to Fisher grade of subarachnoid hemorrhage, Hunt-Hess grade on admission and time of intervention of anterior communicating artery aneurysm. With age, sex, history of hypertension, aneurysm size, stent-assisted embolization. Logistic regression analysis showed that Fisher grade and operative timing were independent factors of symptomatic cerebral vasospasm. Conclusion: according to its influencing factors, select the right intervention time and appropriate intervention method, and obtain the ideal therapeutic effect. Whether stent-assisted embolization is the influencing factor of cerebral vasospasm needs further study.
