Duchenne型肌营养不良合并脊柱侧凸危险因素的Logistic回归分析

【摘要】:目的:探讨Duchenne型肌营养不良合并脊柱侧凸的相关危险因素,为Duchenne型肌营养不良合并脊柱侧凸的病情判断提供依据。方法:选取Duchenne型肌营养不良患者200例,其中并发脊柱侧凸79例为病例组,而121例未并发脊柱侧凸为对照组,采用单因素χ2检验和多因素Logistic回归分析Duchenne型肌营养不良合并脊柱侧凸的相关危险因素。结果:多因素分析显示下肢肌力不对称(OR=3.545,95CI:1.242~10.115),独立行走困难(OR=16.688,95CI:5.036~55.297),未坚持康复锻炼(OR=0.068,95CI:0.015~0.305),跟腱挛缩形成马蹄内翻足(OR=1.597,95CI:0.462~5.524),未使用激素治疗(OR=1.464,95CI:1.437~5.439)。结论:下肢肌力不对称,独立行走困难,未坚持康复锻炼,跟腱挛缩形成马蹄内翻足,未使用激素治疗等是Duchenne型肌营养不良合并脊柱侧凸的独立危险因素,临床上对有上述危险因素的Duchenne型肌营养不良患者应注意预防脊柱侧凸,避免发生脊柱侧凸造成不良后果。
[Abstract]:Objective: to investigate the risk factors of Duchenne muscular dystrophy with scoliosis and to provide evidence for the diagnosis of Duchenne muscular dystrophy with scoliosis. Methods: 200 cases of Duchenne muscular dystrophy were selected, among them 79 cases were complicated with scoliosis and 121 cases were not complicated with scoliosis as control group. Univariate 蠂 2 test and multivariate logistic regression analysis were used to analyze the risk factors of Duchenne muscular dystrophy with scoliosis. Results: multivariate analysis showed that the lower extremity muscle strength was asymmetrical (ORN 3.54595 CI: 1.242J 10.115), the difficulty of independent walking was difficult (ORL 16.68895 CI: 5.03655.297), the rehabilitation exercise was not persisted (ORN 0.068 95CI0.0150.305), the contracture of Achilles tendon resulted in equinovarus varus (OR1.59795 CI: 0.4622.524), and no hormone therapy was used (ORL 1.46495 CI: 1.4375.439). Conclusion: the independent risk factors of Duchenne muscular dystrophy combined with scoliosis are asymmetry of lower limb muscle strength, difficulty of independent walking, failure to insist on rehabilitation exercise, formation of equinovarus foot caused by Achilles tendon contracture and no hormone therapy. Prevention of scoliosis should be paid attention to in patients with Duchenne muscular dystrophy with the above mentioned risk factors, and the adverse consequences of scoliosis should be avoided.
