国内颈椎病误诊的文献计量学分析

【摘要】：目的:通过对颈椎病误诊文献的分析,了解颈椎病误诊的历史沿革和发展趋势。方法:对1983—2014年收录在相关期刊论文(CNKI)、万方数据资源系统和维普中文期刊数据库(VIP)中的颈椎病误诊文献进行研究,分析颈椎病误诊的年代特征、不同分型颈椎病的误诊情况,以及易与颈椎病相混淆的疾病类型。结果:(1)1983—2014年国内的颈椎病误诊病例报道文献共595篇,误诊病例数共5687例。其中颈椎病误诊为其他疾病的文献232篇,病例数为3936例;其他疾病误诊为颈椎病的文献372篇,病例数为1751例。(2)颈椎病的误诊病例从1983年开始呈波动性增长,2008年达到高峰期,之后开始缓慢减少;在颈椎病的误诊病例中,2004年之前主要以"颈椎病误诊为其他疾病"为主,而"其他疾病误诊为颈椎病"的病例相对较少,2005年之后"其他疾病误诊为颈椎病"的病例开始迅速增加。(3)在颈椎病的误诊病例中,报道最多的是"不分型颈椎病",其次是"脊髓型颈椎病"。(4)与颈椎病相混淆的疾病有"肿瘤"、"心脑血管系统疾病"、"神经系统疾病"和"运动系统疾病"等8种类型的疾病。结论:颈椎病的误诊由来已久,与颈椎病相混淆的疾病种类具有多系统性的特点,近年来临床对颈椎病的诊断有扩大化的倾向,亟需临床医师进一步掌握颈椎病与其他疾病的鉴别诊断,反思颈椎病的定义与分型,才能减少颈椎病的误诊误治。
[Abstract]:Objective: to understand the history and trend of misdiagnosis of cervical spondylosis by analyzing the misdiagnosis literature. Methods: the misdiagnosis literatures of cervical spondylosis in CNKI, Wanfang data resource system and VIP database from 1983 to 2014 were studied, and the age characteristics of misdiagnosis of cervical spondylosis were analyzed. The misdiagnosis of different types of cervical spondylopathy and the types of diseases easily confused with cervical spondylosis. Results: (1) A total of 595 misdiagnosed cases of cervical spondylosis were reported from 1983 to 2014 in China, with 5687 misdiagnosed cases. Among them, there were 3936 cases of cervical spondylopathy misdiagnosed as other diseases, and 372 cases of other diseases misdiagnosed as cervical spondylosis, 1751 cases. (2) the misdiagnosis of cervical spondylosis increased from 1983 to 2008, and reached its peak in 2008. The number of misdiagnosed cases of cervical spondylosis was mainly "misdiagnosed as other diseases" before 2004. However, there are relatively few cases of "other diseases misdiagnosed as cervical spondylosis". After 2005, the number of cases of "other diseases misdiagnosed as cervical spondylosis" began to increase rapidly. (3) among the misdiagnosed cases of cervical spondylosis, The most reported diseases were "cervical spondylopathy without typing", followed by "cervical spondylopathy of myelopathy". (4) the diseases confused with cervical spondylosis were "tumor", "cardio-cerebrovascular system disease", "nervous system disease" and "motor system disease". Conclusion: the misdiagnosis of cervical spondylopathy has a long history, and the types of diseases confused with cervical spondylosis have many systematic characteristics. In recent years, the diagnosis of cervical spondylosis tends to expand. In order to reduce misdiagnosis and mistreatment of cervical spondylosis, it is necessary for clinicians to master the differential diagnosis between cervical spondylosis and other diseases, and to reflect on the definition and classification of cervical spondylosis.
