三踝骨折切开复位内固定两种手术顺序的对比研究

【摘要】：目的:为了提高三踝骨折手术治疗的优良率,对两种不同的手术复位固定顺序(即外踝-后踝-内踝与后踝-外踝-内踝)的疗效进行对比分析,以期望今后对该类骨折手术治疗的过程中能够更加合理的选择复位固定顺序提供实验数据。从而提高三踝骨折手术治疗的优良率。方法:按踝关节骨折AO分类标准挑选30例三踝骨折病例,入选病例均来自骨科住院收治的踝关节骨折患者。其中女患者13人,男患者17人,分为Ⅰ、Ⅱ两组,每组15例病人,随机采用两种不同的骨折复位顺序。对Ⅰ组患者采用先后踝再外踝最后内踝的显露及内固定顺序,对Ⅱ组采用先外踝再后踝最后内踝的顺序。术中观测每位患者的手术用时、手术中总出血量、手术中后踝的调整次数。术后记录患者的在院时间和达到临床愈合标准的时间。并于术后6个月后对全部30例患者进行逐一随访,并根据踝-后足评分标准对踝关节功能进行评分。评分依据为踝-后足评分标准,该评分标准由美国足踝外科协会制定结果:两种复位顺序在手术平均用时、后踝复位次数、术中出血量方面均有统计学差异(P0.05)。在住院时间、临床愈合时间、踝后足功能评分方面无统计学差异(P0.05)。结论:后踝-外踝-内踝的显露复位固定顺序与外踝-后踝-内踝的显露复位固定顺序相比,能够减少手术时间、后踝复位次数及术中出血量,但在住院时间、临床愈合时间以及后期关节功能评分方面无明显优势。
[Abstract]:Objective: to improve the excellent and good rate of surgical treatment of trimalleolar fractures, two different surgical reduction and fixation sequences (lateral malleolus posterior malleolus medial malleolus and posterior malleolus lateral malleolus medial malleolus) were compared and analyzed.
