腰椎间盘突出症严重程度与腰椎曲度相关性的影像学研究

【摘要】：目的探讨腰椎间盘突出症严重程度与腰椎曲度的关系。方法随机抽取52例保守治疗无效需开放手术治疗的腰椎间盘突出症患者作为手术组,同时从保守治疗有效腰椎间盘突出症患者、体检科正常人员中选取年龄及性别相匹配的52例分别作为保守治疗组、正常组,3组皆在腰椎侧位X线片上测量腰椎曲度的腰椎曲线指数、腰椎前凸角度、骶骨倾斜角、Harrison椎体后缘切线角,并对所测量数值作统计学分析。结果腰椎曲线指数、腰椎前凸角度、骶骨倾斜角、Harrison椎体后缘切线角在3组比较均差异有统计学意义(P0.01),手术组的4种测量值均比正常组明显减小(P0.01),腰椎前凸角度及骶骨倾斜角比保守治疗组减小(P0.05),但腰椎曲线指数及Harrison椎体后缘切线角与保守治疗组差异无统计学意义(P0.05),保守治疗组的腰椎曲线指数、腰椎前凸角度、Harrison椎体后缘切线角均比正常组小(P0.01),但骶骨倾斜角与正常组差异无统计学意义(P=0.102)。结论腰椎盘突出症越严重患者其腰椎曲度异常改变也越明显,在临床治疗中应意识到腰椎曲度的重要性。
[Abstract]:Objective to investigate the relationship between the severity of lumbar disc herniation and lumbar curvature. Methods A total of 52 patients with lumbar disc herniation were randomly selected as the operation group, and the patients with effective lumbar disc herniation were treated with conservative treatment. 52 patients with age and sex matching were selected as conservative treatment group. The lumbar curve index and lumbar kyphosis angle of lumbar curvature were measured on lateral radiographs in normal group. The sacral obliquity angle and the posterior edge tangent angle of Harrison vertebra were analyzed statistically. Results the curve index of lumbar vertebrae, the angle of lumbar kyphosis, There were significant differences in the posterior edge tangential angle of the sacrum angle between the three groups (P0.01). The four measurement values of the operation group were significantly lower than those of the normal group (P0.01), the lumbar kyphosis angle and the sacral angle of the treatment group were lower than that of the conservative treatment group (P0.05), but the lumbar waist angle of the operation group was lower than that of the conservative treatment group (P0.05). There was no significant difference in vertebral curve index and Harrison posterior edge tangential angle between the conservative treatment group and the conservative treatment group (P0.05). The lumbar spine curve index in the conservative treatment group was higher than that in the conservative treatment group. The angle of posterior edge tangent of Harrison vertebral body was smaller than that of normal group (P0.01), but there was no significant difference between the angle of sacral obliquity and that of normal group (P0. 102). Conclusion the more severe the lumbar disc herniation, the more obvious the abnormal changes of lumbar curvature are, and the importance of lumbar curvature should be recognized in clinical treatment.
