不同粘度骨水泥PVP治疗骨质疏松椎体压缩骨折的弥散程度分析

【摘要】：目的 :观察椎体成形术中使用不同粘度骨水泥后椎体弥散程度的差异。方法 :95例胸腰椎新鲜骨折患者,随机分组,A组46例和B组49例,A组使用低粘度骨水泥,B组使用高粘度骨水泥。均行椎体成形术,术后均进行CT扫描,计算椎体内骨水泥的弥散体积及弥散系数,并记录骨水泥渗漏情况。结果:两组研究对象在单个椎体手术时间、骨水泥注入量、骨水泥弥散体积、弥散系数及渗漏率上差异无统计学意义,但B组有着相对较低的渗漏率,而在骨水泥分布形状上两组差异有统计学意义(P0.05)。结论 :使用高、低粘度骨水泥的椎体成形术均能有效弥散到上下终板和椎体中心,具有相似的弥散系数,但高粘度骨水泥弥散多呈实心团块状,而低粘度骨水泥多呈蜂窝状,且高粘度骨水泥由于渗漏率较低,具有相对良好的安全性。
[Abstract]:Objective: to observe the diffusivity of vertebral body after using different viscosity bone cement in vertebroplasty. Methods Ninety-five patients with fresh thoracolumbar fractures were randomly divided into two groups: group A (n = 46) and group B (n = 49) treated with low viscosity bone cement. All patients underwent vertebroplasty and CT scans were performed after operation to calculate the diffusion volume and diffusion coefficient of bone cement in the vertebral body and to record the leakage of bone cement. Results: there were no significant differences between the two groups in single vertebral operation time, bone cement injection, bone cement dispersion volume, diffusion coefficient and leakage rate, but in group B there was a relatively low leakage rate. But in the bone cement distribution shape two groups have the statistical significance (P0.05). Conclusion: vertebroplasty with high or low viscosity bone cement can effectively diffuse to the upper and lower endplate and the center of vertebral body, and has similar diffusion coefficient, but the dispersion of high viscosity bone cement is mostly solid mass, while low viscosity bone cement is honeycomb. And high viscosity bone cement has relatively good safety because of its low leakage rate.
