石膏固定和空心螺钉内固定修复腕部舟状骨新鲜骨折:功能恢复比较

【摘要】：目的：比较分析石膏固定和空心螺钉内固定修复腕部舟状骨新鲜骨折的功能恢复情况。方法：纳入新疆医科大学第一附属医院2012年3月至2014年7月收治的腕部舟状骨新鲜骨折22例,患者均为新鲜舟状骨腰部骨折,受伤时间2h-12d。患者术前X射线拍片示：未见腕部舟状骨塌陷、月骨周围脱位、缺血性骨坏死及骨性改变等表现。所有病例均为有移位、不稳定性骨折。根据患者意愿及手术方法选择等分为石膏组和空心螺钉组进行疗效对比分析,每组11例。随访时对两组患者进行Cooney腕关节功能评分。两组患者年龄分布、性别差异无显著性意义(P0.05)。结果：随访3-6个月对比分析石膏组和空心螺钉组患者腕关节功能Cooney评分,空心螺钉组优9例,良1例,可1例,差0例：石膏组优1例,良5例,可3例,差2例,空心螺钉组患者治疗后优良率为91%,石膏组患者治疗后优良率为55%,空心螺钉组显著高于石膏组(t=4.817,P0.05)。结论：虽然石膏固定在腕部舟状骨新鲜骨折上具有一定的疗效,但在治疗后腕关节功能恢复方面,新鲜腕部舟状骨骨折切开复位空心螺钉内固定具有明显的疗效并且患者腕关节功能恢复较快。因此与石膏固定相比,切开复位空心螺钉内固定修复腕部新鲜舟状骨骨折更具优势。
[Abstract]:Objective: To compare and analyze the functional recovery of fresh fractures of the scaphoid in the wrist repaired with plaster fixation and hollow screw internal fixation. Methods: 22 fresh scaphoid fractures were treated in the First Affiliated Hospital of Xinjiang Medical University from March 2012 to July 2014, all of them were fresh canoe bone fracture and 2h-12d. patients were injured. The anterior X ray film showed no scaphoid collapse of the wrist, dislocation of the bone around the moon, ischemic osteonecrosis and bone change. All cases were displaced and unstable fractures. According to the patient's wishes and surgical methods, the curative effect pairs were divided into the gypsum group and the hollow screw group, with 11 cases in each group. The two groups were followed up. Cooney wrist joint function score. There was no significant gender difference in age distribution between the two groups (P0.05). Results: after 3-6 months of follow-up, the wrist joint function Cooney score of the plaster group and the hollow screw group was compared and analyzed. There were 9 cases of hollow screw group, 1 good cases, 1 cases, 0 poor cases, good 5 cases, 3 cases, 2 poor cases, and hollow screw group. The good rate after treatment was 91%, the good rate after the treatment in the plaster group was 55%, and the hollow screw group was significantly higher than the plaster group (t=4.817, P0.05). Conclusion: Although the plaster fixation has a certain effect on the wrist scaphoid bone fresh fracture, the fresh wrist scaphoid fracture is open and repositioned with hollow screw in the wrist joint function recovery side. The fixation has a significant effect and the function of the wrist joint is faster. Therefore, compared with the plaster fixation, open reduction and internal fixation with hollow screw for the repair of the fresh scaphoid fracture of the wrist has a better advantage.
