PVP联合RFA对骨质疏松性椎体压缩性骨折患者术后疼痛及生活质量的影响

【摘要】：目的探讨经皮椎体成形术(PVP)联合射频消融术(RFA)对骨质疏松性压缩性脊柱骨折患者术后疼痛及生活质量的影响。方法选取骨质疏松性脊柱压缩性骨折患者77例,按治疗方法分为对照组35例、观察组42例。对照组单纯采用PVP,观察组在PVP基础上联合RFA。记录术中肺栓塞、骨水泥渗漏等并发症情况。术后1个月评价临床疗效。术前、术后l周、3个月、6个月及12个月时采用目测类比(VAS)评分及Barthel指数进行疼痛及生活质量评分。结果对照组、观察组有效率分别为91.43%、92.86%,两组有效率差异无统计学意义(P0.05)。与治疗前比较,两组治疗后VAS评分均下降、Barthel指数均升高(P均0.05)。观察组术后3个月、术后6个月及术后12个月时VAS评分均低于对照组,Barthel指数均高于对照组(P均0.05)。两组术中均无肺栓塞等严重并发症发生。结论 PVP联合RFA治疗骨质疏松性脊柱压缩性骨折可明显缓解患者术后疼痛,提高生活质量。
[Abstract]:Objective to investigate the effect of percutaneous vertebroplasty (PVP) combined with radiofrequency ablation (RFA) on postoperative pain and quality of life (QOL) in patients with osteoporotic vertebral compression fracture. Methods Seventy-seven patients with osteoporotic spinal compression fracture were divided into control group (n = 35) and observation group (n = 42). The control group was treated with PVP alone, and the observation group was combined with RFA on the basis of PVP. The complications of pulmonary embolism and bone cement leakage were recorded. The clinical effect was evaluated 1 month after operation. Preoperative, 1 week, 3 months, 6 months and 12 months after operation, pain and quality of life scores were evaluated by visual analogy (VAS) score and Barthel index. Results in the control group, the effective rate of the observation group was 91.43 and 92.86, respectively. There was no significant difference between the two groups in the effective rate (P0.05). Compared with before treatment, VAS scores decreased and Barthel index increased after treatment in both groups (P < 0. 05). The VAS score in the observation group was lower than that in the control group at 3 months, 6 months and 12 months after operation (P < 0. 05). No serious complications such as pulmonary embolism occurred in both groups. Conclusion PVP combined with RFA can significantly relieve postoperative pain and improve quality of life in patients with osteoporotic spinal compression fracture.
