骨髓干细胞和伞状支撑骨移植术治疗股骨头坏死疗效观察

【摘要】：目的探讨骨髓干细胞移植联合伞状支撑骨移植术治疗股骨头坏死(osteonecrosis of the femoral head,ONFH)的临床效果。方法 ONFH患者48例(58髋),其中30例(36髋)采用骨髓干细胞和伞状支撑骨移植术为观察组,18例(22髋)行单纯股方肌骨柱移植术为对照组;比较2组术后百分法评分和影像学检查结果。结果观察组与对照组术前百分法评分[(56.06±6.88)、(55.54±6.36)分]比较差异无统计学意义(P0.05);术后随访18个月,2组百分法评分均较术前提高,且观察组评分[(87.47±5.66)分]高于对照组[(76.14±7.57)分],治疗优良率(91.67%)高于对照组(72.7%)(P0.05);观察组术后植骨愈合时间[(3.9±0.5)个月]和MRI坏死面积百分比[(9.19±5.84)%]低于对照组[(5.2±0.6)个月、(14.05±5.96)%],差异均有统计学意义(P0.05)。结论骨髓干细胞和伞状支撑骨移植术治疗ONFH可加速成骨,更好恢复股骨头的生物力学、防止股骨头塌陷,近期效果良好。
[Abstract]:Objective to investigate the clinical effect of bone marrow stem cell transplantation combined with umbrella support bone graft in the treatment of femoral head osteonecrosis (osteonecrosis of the femoral head ONFH). Methods 48 patients (58 hips) with ONFH were treated with bone marrow stem cells (BMSCs) and umbrella-supported bone graft (UBSO) in 30 cases (36 hips) as the control group (n = 18). The score and imaging results of the two groups were compared. Results there was no significant difference in the preoperative score between the observation group and the control group [(56.06 卤6.88), (55.54 卤6.36)] (P0.05). The score of the observation group [(87.47 卤5.66)] was higher than that of the control group [(76.14 卤7.57)], the excellent and good rate of treatment (91.67%) was higher than that of the control group (72.7%) (P0.05), the time of bone graft healing [(3.9 卤0.5) months] and the percentage of MRI necrosis area [(9.19 卤5.84)%] in the observation group were significantly lower than those in the control group [(5.2 卤0.6) months, (14.05 卤5.96)%] (P0.05). Conclusion Bone marrow stem cells and umbrella bone graft can accelerate osteogenesis, recover biomechanics of femoral head and prevent femoral head from collapsing.
