微创旋切术与开放性手术治疗乳腺良性肿瘤的临床疗效观察

【摘要】：目的对比由安珂微创系统治疗乳腺良性肿物与开放性手术治疗的临床效果,使手术方法更精细化、更安全、更高效、更美观并探寻微创术在乳腺疾病诊疗方面的潜在价值。方法本研究通过回顾性研究分析的方法搜集2014.10-2016.10青海大学附属医院乳腺甲状腺外科收治的乳腺良性肿块患者共406例,将行微创术的197例和开放术的209例患者分别设为观察组、对照组,比较两组间患者的年龄、病程和肿物最大直径等一般资料无统计学差异后,获得可比性。记录两组患者在术中如切口平均总长度、手术时间和出血量及术后如并发症等数据资料进行统计分析。结果观察组和开放组所有病灶完全切除,未见残留及复发。观察组与开放组在手术时间比较[(15.39±6.20)min vs(32.55±6.36)min,t=27.518,P=0.00],P0.05,差异有统计学意义;在术中出血量方面[(10.65±3.19)ml vs(15.47±4.64)ml,t=12.263,P=0.00],两组数据比较P0.05,差异有统计学意义;在平均总长度方面[(3.0±0.01)mm vs(22.51±12.53)mm,t=22.512,P=0.00],P0.05,差异有统计学意义;VAS疼痛评分两组对比[(2.41±0.47)分vs(4.27±1.38)分,t=18.307,P=0.00],P0.05,差异有统计学意义;观察组和对照组满意率为94.42%和72.24%,经对比有统计学差异。术后并发症情况(χ2=0.849,P=0.357),P0.05,差异无统计学意义。结论安珂微创系统治疗乳腺良性肿瘤对比开放术具有手术时间短、出血量少、美容效果好等优点,在确定疗效前提下,符合外科手术微创化趋势。
[Abstract]:Objective to compare the clinical effects of An Ke minimally invasive system in the treatment of benign breast masses and open surgery, so as to make the operation more refined, safe, efficient and more beautiful, and to explore the potential value of minimally invasive surgery in the diagnosis and treatment of breast diseases. Methods A total of 406 patients with breast benign masses admitted to the Department of Breast thyroid surgery in Qinghai University affiliated Hospital on October 10, 2014.2014-2016.2014.197 patients with minimally invasive surgery and 209 patients with open surgery were divided into observation group. In the control group, there were no statistical differences in age, course of disease and maximum diameter of tumor between the two groups. The average total length of incision, operative time, blood loss and postoperative complications were recorded and analyzed statistically in both groups. Results all the lesions in the observation group and the open group were completely resected without residual or recurrence. The operative time in the observation group and the open group [(15.39 卤6.20) min vs (32.55 卤6.36) min] was significantly higher than that in the open group [(10.65 卤3.19) ml vs (15.47 卤4.64) ml vs (12.263P0.00], and the difference between the two groups was statistically significant. The average total length [(3.0 卤0. 01) mm vs (22.51 卤12. 53) mm vs (] was significantly different between the two groups [(2. 41 卤0. 47) vs (4. 27 卤1. 38) vs 18. 307P0.00] P0.05.The satisfaction rates of the observation group and the control group were 94.42% and 72.24%, respectively. There was no significant difference in postoperative complications (蠂 ~ 2 ~ (0.849) P ~ (0.357) and P ~ (0.05). Conclusion An Ke minimally invasive system for the treatment of benign breast tumors has the advantages of short operation time, less bleeding, good cosmetic effect, etc., which accords with the trend of minimally invasive surgery on the premise of determining the curative effect.
