内髁Hoffa骨折合并前交叉韧带断裂的诊疗经验总结

【摘要】：目的总结内髁Hoffa骨折合并前交叉韧带断裂的诊疗经验。方法 2012年1月—2016年2月,收治3例外伤致内髁Hoffa骨折合并前交叉韧带断裂患者。男2例,女1例;年龄分别为59、43、55岁。伤后至入院时间为2、3、1 h。采用空心螺钉固定骨折块,关节镜下一期前交叉韧带重建治疗。结果术后患者切口均Ⅰ期愈合。3例患者分别获随访10、9、12个月。X线片复查示,骨折均愈合,愈合时间分别为12、14、16周;随访期间无创伤性关节炎发生。末次随访时,参照Letenneur提出的Hoff骨折术后功能恢复评价系统,膝关节功能均达优。结论对于内髁Hoffa骨折合并前交叉韧带断裂,切开复位空心螺钉内固定联合膝关节镜下前交叉韧带重建术是一种有效、安全方法。
[Abstract]:Objective to summarize the experience of diagnosis and treatment of medial condylar Hoffa fracture with anterior cruciate ligament fracture. Methods from January 2012 to February 2016, three patients with Hoffa fracture of medial condyle associated with anterior cruciate ligament fracture were treated. There were 2 males and 1 females, aged 59 / 43 and 55 years old respectively. The time from injury to admission was 2 minutes and 3 minutes. First stage anterior cruciate ligament reconstruction under arthroscopy was performed with hollow screw fixation. Results Thirty three patients with primary wound healing were followed up for 10 ~ 9 months and 12 months. X-ray examination showed that the fracture healed for 12 ~ 14 ~ 16 weeks, and no traumatic arthritis occurred during the follow-up period. At the last follow-up, the function of knee joint was excellent according to the evaluation system of postoperative function of Hoff fracture proposed by Letenneur. Conclusion for Hoffa fracture of medial condyle with anterior cruciate ligament rupture, open reduction and hollow screw fixation combined with anterior cruciate ligament reconstruction under knee arthroscopy is an effective and safe method.
