MRS定量评价腰椎间盘退变

【摘要】：目的探讨MRS技术定量评价腰椎间盘退变程度的价值。方法对82例腰痛患者行腰椎MR常规扫描,采用T2WI矢状位图像进行Pfirrmann分级;采用MRS技术检测腰椎间盘髓核的氮-乙酰氨基(N-acetyl)峰下面积、水(Water)峰下面积及二者峰下面积比值,分析MRS测量值与Pfirrmann退变分级、患者年龄间的相关性。结果 82例患者中,204个椎间盘可获得MRS测量值,其中Ⅱ、Ⅲ、Ⅳ、Ⅴ级椎间盘数目分别为89个、73个、39个、3个,髓核区N-acetyl峰下面积、Water峰下面积、N-acetyl/Water峰下面积比值与腰椎间盘Pfirrmann分级呈负相关(r_s=-0.460、-0.204、-0.526,P均0.05)。30岁、30~39岁、40~49岁、50~59岁、59岁的患者中,取得MRS测量值的椎间盘数目分别为62个、25个、37个、51个、29个,腰椎间盘髓核区N-acetyl峰下面积、N-acetyl/Water峰下面积比值与年龄段呈负相关(r_s=-0.247、-0.385,P均0.05)。结论 MRS技术可用以定量检测腰椎间盘的退变程度。
